This is a primarily in‑office position focused on meaningful, face‑to‑face clinical visits with hospice patients while collaborating closely with physicians and interdisciplinary care teams. In this role, you will play a vital part in ensuring exceptional end‑of‑life care by conducting required hospice face‑to‑face evaluations and supporting high‑quality, compliant, and compassionate patient care across our service region.
