Position Summary The Admissions Licensed Practical Nurse (LPN) supports the admissions process and facilitates timely access to care by providing a range of pre-admission and admission services. The LPN collaborates with internal and external partners, including hospitals, long-term care (LTC) facilities, and assisted living facilities (ALFs), to resolve barriers to hospice admission. This role provides information and education about hospice services, assists in addressing advanced directive concerns, and ensures a smooth admission process while advocating for patient and family needs. Employees assigned this job title must be willing and able to flex their work location between the various Hospitals and assist with home admissions as needed.
