Interdisciplinary Collaboration Collaborate closely with physicians, social workers, chaplains, aides, and other members of the care team. Communicate changes in patient condition and coordinate appropriate interventions. Participate in interdisciplinary team meetings and care planning discussions. Quality & Compliance Adhere to hospice, state, federal, and regulatory requirements. Follow infection prevention and control practices. Participate in quality improvement and performance initiatives.
