The Care Promoter 1 is an unlicensed medical professional cross-trained to perform routine administrative and clinical tasks to keep the medical centers running smoothly. The duties of Care Promoter 1 vary from setting to setting, depending on the size, location and type. In addition to administrative duties, Care Promoters perform clinical duties under direct physician supervision in accordance with state medical practice acts. The Care Promoter 1 is an integral member of a Physician-lead Care Team that is focused on providing excellent and comprehensive primary care for a specific population of patients. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: Measures and records patient vital signs; records patient interview and medical history. Catalogs and communicates patient needs to the PCP, Clinician, Specialist and/or other clinical personnel. Prepares patients for examinations and performs routine screening tests; assists physician with exams; explains treatment procedures and physicians' instructions to patient. Observes patients and reports any changes in patient conditions to the PCP, Clinician, Specialist and/or other clinical personnel. Perform non-critical medical and therapeutic procedures based on medical instructions. Collects phlebotomy and other lab specimens; performs basic waived lab tests. Prepares and administers medications; changes dressings, applies bandages, removes sutures and other first aid procedures; uses CPR skills when necessary, all under physician supervision. Maintains supplies, equipment, stocks and sterilizes instruments; practices OSHA safety standards; performs accurate, legal, and ethical documentation at all times. Processes patient phone messages, returning calls and routing them to other team members as appropriate; calls patients to obtain and relay pertinent information for the physician. Upon physician approval and authorization, calls in prescriptions to the pharmacy. Uses communication skills with appropriate medical terminology; and follows appropriate legal and ethical professional conduct; authorized individuals will use the ChenMed Rx system to support physician medication dispensing. Provides health coaching to a defined group of patients to support healthy lifestyle choices. Follows up with coached patients via weekly calls. Performs other duties as assigned and modified at manager’s discretion. This includes the ability to establish rapport, systematically gather data, determine the readiness of an individual for treatment and change and apply accepted criteria for diagnosis of substance use disorders. Screen for co-occurring Primary Mental Health issues. Screen for Substance Use Disorders (SUD) including psychoactive substance toxicity, intoxication, and withdrawal symptoms. Must have knowledge of medications, assessment for medication complications, detoxification protocols and prescribing medications in general Screen for danger to self or others with an evidenced based assessment tool—the Columbia Suicide Screening Rating Scale (CSSRS) and the Patient Health Questionnaire-9 (PHQ-9). Analyze and interpret data to determine treatment recommendations and priorities. Screen for nutrition deficiencies and make interventions or refer for a nutrition consultation as appropriate. Screen for pain and understands treatment strategies for pain including pharmacologic and nonpharmacologic approaches. Strategies should reflect an individualized approach based on the individual’s current presentation, medical judgment and the risks and benefits associated with strategies, including potential risk of dependency, addiction, and abuse. Demonstrates proficiency using the electronic health record with all required documentation. ensuring compliance with all company policies and procedures, state regulation, Joint Commission standards. These records will be completed and authenticated in a timely manner per policy. This includes but is not limited to the following electronic record documents: A. Psychiatric Evaluation B. Physical Examinations C. Medical Progress Notes D. Medical Discharge Summaries E. Medication and ancillary orders F. Allergies G. Diagnosis and updating diagnosis H. Laboratory Date interpretation and documentation I.HIV results positive or negative must be reviewed and documented in the medical record. STD positive results must be communicated to nursing so they can report to the State Health dept as applicable and reviewed with patient to consider treatment options. Understands ASAM criteria and Medical Needs Criteria provided and will document decision making and treatment planning accordingly. Understands the On-Call responsibilities: will provide contact and back-up numbers, being responsive and available when assigned on-call duties. Must be within one hour of facility when on-call. Other required tasks with the Medical Director: Develops cost-effective methods to provide medically necessary therapeutic services. Assists in formulating and supporting the highest standards of patient care. Review all level of care changes and provides peer reviews when required. Provides support to Nurse Practitioners, Physician Assistants and students as required. Understands and abides by all safety/risk, infection control and other guidelines as required. If Non-Exempt, clocks into ADP at the designated time frames. Performs other tasks and duties as assigned. Graduated from an accredited Nursing Master’s Program with a Family Nurse Practitioner or higher and a Psychiatric Nurse practice degree. Experience in Addiction Medicine/ Primary Behavioral Health preferred. Familiarity with JCAHO and other indicated healthcare standards is preferred. Familiarity with ASAM/ ASAM Criteria and other indicated healthcare standards is preferred. Application of Continuous Quality Improvement and Performance Improvement Team philosophy and ability to develop and apply organizational strategies. Communication skills to relate to all levels of facility staff. Self-motivated and confident. Depending on the state practicing, may need a protocol agreement to be completed by the NP and Medical Director. Licensed Nurse with Master’s Degree or higher to practice as a Family Nurse Practitioner and a Psychiatric Nurse Practitioner (Psych NP) in the appropriate state, as applicable, required. Must have a DEA to prescribe controlled medications and have had training on Buprenorphine Prescribing as outlined by the DEA in the state(s) working. DEA-authorized prescribers will be required to demonstrate that they have completed at least eight (8) hours of educational training on the treatment and management of patients with opioid or other substance use disorders. Depending on the state practicing, may need a protocol agreement to be completed by the NP and Medical Director. Depending on the state, the DEA may not be able to be obtained until the protocol has been approved by the local nursing board. Job Description Nemours Children Health is currently seeking a Neonatal Nurse Practitioner to work at HCA Florida Osceola Hospital. Nemours Children’s Health has developed a strategic partnership with HCA Florida Osceola Hospital to provide neonatal care in their 18 bed level III NICU. The program delivers ~1200 babies a year with a OBGYN residency program and a robust MFM service. HCA Florida Osceola Hospital serves as the high-risk delivery center for the Nemours Children’s Hospital Florida referring pregnant women carrying infants with congenital disorders from across the state. The successful candidate would join a team of highly motivated, energetic Neonatologists and experienced Nemours Neonatal Advanced Practice Providers. Responsibilities include rounding on well babies, attending high risk deliveries, teaching and mentoring Nemours Pediatric residents and University of Central Florida medical students. The level III NICU is staffed with 24/7 in house Nemours Advance Practice Providers with a total of 3 Neonatologists who round and take call from home. Job Requirements POSITION SUMMARY The Registered Nurse (RN) provides direct and in-direct patient care by adhering to the nursing process in a collaborative environment, promoting optimal patient outcomes through evidence-based practice, care coordination, and patient and family engagement. PRIMARY ACCOUNTABILITIES Provides safe and effective nursing care based on assessment, planning, implementation, and evaluation of patient needs across the care continuum. Administers medications, perform treatments, and document care activities in accordance with Health First’s policies and procedures. Collaborates with interdisciplinary team members to coordinate care, participate in rounds, and support discharge planning. Applies critical thinking to promptly identify changes in patient condition and initiate the appropriate interventions. Educates patients and families using teach-back to promote engagement in care and safe transitions. Adheres to safety protocols and infection prevention practices to protect patients and Associates. Utilizes Health First technology systems to enter clinical documentation and care plans. Participates in quality improvement, patient safety, and High Reliability Organization (HRO) initiatives. Supports precepting, mentoring, and onboarding of new Associates and students as appropriate. Promotes a culture of purpose, rounding, and communication at the bedside (e.g., whiteboard use, bedside shift report). Work Experience MINIMUM QUALIFICATIONS Education: Associate’s degree in Nursing (ASN) or Nursing Diploma. Work Experience: Meets unit specific care requirements; new graduates taken under consideration based on the needs of the unit. Licensure: Registered Nurse (RN) licensure in the State of Florida or endorsement. Certification: American Heart Association Basic Life Support (AHA BLS) Healthcare Provider Completion Card prior to start date and maintained. Additional certifications to meet unit specific care requirements (e.g., ACLS, PALS, NRP, NIHSS). Skills/Knowledge/Abilities: Strong clinical assessment and critical thinking skills. Proficiency in EMR systems and patient documentation. Demonstrated initiative in quality and patient safety improvements. Ability to work autonomously and collaboratively in complex environments. Must maintains compliance with competency validations and continuing education. Demonstrate flexibility, reliability, and a commitment to a collaborative work environment. PREFERRED QUALIFICATIONS Education: Bachelor’s degree in Nursing (BSN). Work Experience: One (1) year or more of acute care or specialty setting. Certification: National certification in specialty area (e.g., CCRN, CMSRN, CEN). PHYSICAL REQUIREMENTS Physically agile; requires walking, standing, running, bending, stooping, kneeling, climbing, or crouching majority of the shift. Heavy work that frequently includes lifting or moving objects up to 100 pounds or more with or without assistance. May require moving self in different positions to accomplish tasks in various environments including tight and confined spaces, including reaching out and above shoulders or overhead. May lift, turn, pull, push, or ambulate people and/or equipment with or without assistance up to 90% of the assigned shift. May be exposed to inside environments and outside environments with extreme temperatures, air contamination, dirt, grease, or water. May require working with biohazards such as blood/body fluids and airborne particles following OSHA and CDC guidelines. May operate various types of manual and motorized equipment used in the transportation of materials. May be exposed to intense noise or vibration and awkward or confined work position. Communicating with others to exchange information. Visual acuity and hand-eye coordination to perform tasks. Workspace may vary from open to confined. May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
