MAJOR FUNCTIONS: Under general direction, the Nurse Practitioner is responsible under direction of a physician for providing general medical care, preventative care and treatment to patients in one of the agency’s clinics. Job description: We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our Social Service Health Care Team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive patient care and coordinating services for the people we support.
