Description RN/Case Manager MSH Case Management PT Days The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of patients to determine the appropriateness of the Level of Care, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities 1. Principle Duties and responsibilities include, but are not limited to: a. Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. b. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. c. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. d. Documents all clinical reviews in Case Management Documentation system. e. Supports the mission, vision, philosophy and goals of the Medical Center. 2. Case Management: The CM process will include: a. Assessment of the patients clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. b. Identification and documentation of variances affecting the LOS and the discharge planning process. c. Conducts follow-up of any delays in treatment or reporting of results. d. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. e. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. f. Documentation of outcomes achieved and identified internal and external barriers. g. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. h. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. i. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. 3. Utilization Review: a. Maintains a working knowledge of the UR (Utilization Review) requirements of each payor within the patient population; b. Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. c. Provides clinical information requested by the managed care companies in accordance with contractual agreements. d. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Obtains authorizations from managed care companies for post-discharge services. c. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. d. Liaison with financial department for current insurance coverage. 5. Collaborates and participates in the appeals process with all members of the interdisciplinary team. 6. Collaborative Relationships: a. Develops and maintains effective working relationships with interdisciplinary team and Case Managers within the managed care organizations. 7. Participates in nursing orientation. 8. Interfaces with other departments in the Medical Center. Qualifications Licensing and Certification Requirements: Education Requirements Bachelors Degree in Nursing; Masters preferred. Experience Requirements Previous experience as in homecare, long term care or utilization review preferred. Discharge Planner or Case Manager preferred. Manager or a minimum of 3-5 years experience as a RN in an acute care setting Certification Requirements: Current NYS RN nursing license. CCM certification preferred. Re-credentialing of CCM status expected. Overview Carries out patient care responsibilities with emphasis on decision-making related to the steps in the nursing process to include assessment, diagnosis, and identification of expected outcomes, planning, implementation and evaluation. Responsibilities The position of Chief Nurse Anesthetist is responsible and accountable to the overall operations and management of the CRNA staff, including hiring, evaluating, and mentoring. Collaborates with the Chair of the Anesthesiology Department in developing, interpreting, and applying department policies, budgets, and long-range plans. Analyzes and evaluates activities to assure established quality patient services and plans appropriate utilization of staff time and capabilities. Coordinates these activities with other hospital departments. The Chief Nurse Anesthetist will demonstrate professional and clinical leadership, expertise, and judgment. Qualifications Graduate of an accredited Nurse Anesthesia Education Program with a Master’s Degree in a related field; or graduation from a program accredited by the Council on Accreditation of Nurse Anesthesia Education programs prior to January 1998 is required. Certified as an APN in the State of NJ is required. A minimum of five (5) years of experience as a nurse anesthetist is required. Responsibilities The primary purpose of the Advanced Practice Nurse (APN) position is to provide high quality patient care. The APN utilizes advanced practice nursing expertise to provide care to patients in the Emergency Department. The OR Technician II performs various duties to assist with surgical procedures and related operating room activities. The OR Technician II delivers care as reflected in the Association of Surgical Technologists’ Recommended Standards of Practice in a manner that is cost-effective without compromising quality of care. The OR Technician II possesses knowledge of the theory and application of sterile and aseptic technique and combines the knowledge of human anatomy, surgical procedures, and implementation tools to facilitate a physician’s performance of invasive, therapeutic, and diagnostic procedures. The OR Technician II demonstrates competency to scrub for minor/major surgical and endoscopic surgical procedures. Must work in air-conditioned environment with temperature of 65-73 degrees and humidity of 45-55%. When assigned to pediatric cases, temperature may be up to 85-90 degrees F. Must wear hospital provided scrub attire while on duty. Potential for cuts, bruises, muscle strains and exposure to contagious disease via and body fluids. Potential for exposure to anesthesia waste gases, ethylene oxide, harsh disinfectants, sterilizing agents, high steam pressure agents and radiation. Responsibilities The primary purpose of the Advanced Practice Nurse position is to participate under the administrative, clinical direction and leadership of the Director of the Neonatology Program and in collaborative relationship in the (Mother-Baby Unit), Neonatal Intensive Care Units (FICN and FIN Nurseries), Labor and Delivery areas with the other healthcare providers. Interacts collaboratively with the nurses and other multidisciplinary team members. Duties are to provide primary care for the high-risk newborn infants in these locations in accordance with the objectives, policies and procedures of the hospital and the New Jersey State Advanced Nurse Practice Act. Participates under the direction of the Director of Neonatology and in a collaborative relationship with Attending Neonatologist, Nurse Manager and Nursing staff in the Intensive Care and Intermediate Care Nurseries and Labor and Delivery Room. Interacts in a collaborative way with a multidisciplinary team as well as with community agencies including DCP&P and community nursing services. Measurement-criteria are "as observed by the Director of Neonatology in collaboration with the Chief APN of the Hospital” for all performance standards, unless otherwise indicated. Qualifications Certified as Advanced Practice Nurse in the State of New Jersey with a minimum of a Master’s Degree in Nursing. Inspect resident’s skin and promptly report any changes to the NCC. Carries out mouth care, hair care and as necessary trim finger and toe nails. Make two hourly checks on incontinent residents and wash and change as necessary. Encourages self-help measures as appropriate so that each child may reach his/her appropriate level of independence. Check and record urine and bowel movements. Ensure that assigned residents are appropriately prepared for school, clinic, recreation and hospital visits as scheduled and accompany/transport them as assigned. Ensure that bedfast residents are repositioned as directed by NCC but not less than two repositions per resident hourly. Performs restorative and rehabilitative activities as instructed and as directed by Plan of Care. Assists in applying safety restraints to residents when directed; Monitors residents in restraints, release restraints and documents according to policy and procedure Ensures that each resident who can use a call bell has one within reach and remind him/her to use it. Assists co-workers with lifting, positioning and other procedures as necessary. Be aware of facility policy on child abuse and/or neglect and promptly report any instances of abusive or inappropriate behaviors towards any resident to the NCC. Knows and follows The Center policy and procedure with regard to infection control including hand washing and types of isolation. Keep resident care area clean and free of clutter. Report any unsafe conditions immediately to NCC. Properly stores and safeguard residents’ clothing and reports too many or too few clothing and or toys to NCC. Feeds residents or teaches residents to feed themselves as appropriate. Responsibilities POSITION SUMMARY The purpose of this position is to support the Nurse Manager to ensure standards of professional practice are developed and maintained to achieve excellence and continuity in resident care. The Assistant Nurse Manager will be responsible for working in a collaborative manner with the Nurse Manager to direct and organize the nursing care for their assigned neighborhoods. The Assistant Nurse Manager will support the leadership, direction, and supervision of nursing staff, including RNs/LPNs, CNAs and Companions. This includes working in collaboration with the Nurse Manager and the neighborhood staff to balance schedules and maintain staffing levels that are consistent with resident needs, including needs as defined by their Plan of Care, activity and daily schedules, outside and in-house clinic visits, and school programs. The Assistant Nurse Manager will also review and/or modify staff assignments as necessary to ensure quality resident care. In addition, the Assistant Nurse Manager will participate in the hiring, interviewing and onboarding process, as well as periodic evaluation of staff that are assigned to their floor. Qualifications Licensed as a Registered Nurse in the State of New Jersey. BLS required. Must maintain valid certification by the American Heart Association in basic life support (BLS). Phlebotomy Certification or IV Certification is required. Proficiency in the English language is required. Minimum of one (1) year Adult Medical/Surgical experience within the past (3) years required. Successful completion of a Progressive Care Course or Progressive Care Certification preferred. BSN preferred. Ability to employ problem solving techniques and critical thinking. Effective interpersonal skills. Ability to organize care/activities of multiple patients. 
Responsibilities POSITION SUMMARY To ensure in collaboration with the Nurse Manager that each neighborhood provide safe, appropriate and goal-directed care and to provide support and direct nursing staff. To be familiar with all policies and procedures. Be able to seek appropriate guidance as needed. Work in collaboration with medical doctors in emergent situations and support and promote family centered care and maintain effective communication with family members. The RN Supervisor will be responsible for working an on-site schedule of a 3-4 days/week including weekends. The ideal candidate has strong clinical skills in change management, emotional intelligence and demonstrated leadership experience in a residential or behavioral health setting. The RN Supervisor will be included in a Nursing Leadership weekend rotation to ensure continuity of systems and protocols are maintained through the entire week. Performs clinical evaluation for the physician on site and reports clinical findings for medical decisions. Guides and supervises the nursing staff in the delivery of nursing care, to ensure continuity of care and to evaluate the quality of total care provided on the evening and night tours of duty. Provides accurate, objective and thorough endorsement of resident status to oncoming management and to clinical status meetings when scheduled Initiates investigations of care issues and family concerns in a timely and thorough manner. This includes, but is not limited to, incident reports, medication errors and abuse and neglect reporting Collaborate with Nurse Manager, supervise and support nursing staff, including performance evaluations, coaching and counseling. Interprets policies and procedures for nursing staff and other services. Provides education and training as needed regarding policy and procedure. Provides input to Team Leaders on a regular basis regarding employee performance on their shift, and intervenes as necessary to provide coaching, counseling and/or progressive discipline. When providing same follows all rules of regulations of the Center, the SDOH and the 1199 bargaining unit. Takes responsibility to arrange coverage for scheduled shift if unable to provide coverage as planned. Displays flexibility and willingness to assure house coverage is provided when requested. Maintains accurate and clear records regarding changes in shift assignment in accordance with rules of the Center and the 1199 bargaining unit contract. Uses SBAR and other tools, such as 24 hours report to objectively document resident/family concerns Manages night medication cabinet and seeks solutions to obtain medication for residents if needed. Attends management meetings as required, this includes Operations Meetings and other Management Training as scheduled Rounds on Neighborhoods for the purpose of obtaining clinical information on residents, to support and educate staff, to assist with family issues and endorses complete information to next shift. Promotes 24 x7 accurate, objective clinical and administrative reports. Follow-up on status of residents transferred to other facilities and reports follow up to oncoming shift, Team Leader and physician Communicates effectively with Center Departments regarding resident/family needs on tour of duty and documents communication in a clear and concise manner. Other related duties as may become necessary or as directed by your supervisor, department director, and/or administrator. Qualifications Education Requirements: Graduate of an accredited RN program B.S. N. (required); M.S.N. preferred RN in the State of New York BCLS certificate PALS certification preferred Completion of courses mandated for continuation of RN license, (ex. Infection Control course; Child Abuse course.) Experience Minimum of two years pediatric experience. Minimum of three years experience in supervisory role. Experience in pediatric and adolescent chronic and restorative nursing modalities preferred. Employee Requirements: Must be able to read, write, speak, and understand the English language fluently. Must meet or exceed all health standards and requirements for the position as established by NYDOH and all other regulatory agencies. Must meet the general health requirements set fourth by the policies of this Center which includes a medical and physical examination. Must be able to push, pull, move, and/or lift a minimum of thirty-five (35) pounds. Perform tasks that may involve exposure to the resident’s blood/body fluids. Must be able to assist in the evacuation of residents, if necessary. Working Conditions: Will work in a well-lighted and ventilated area. Will be subject to frequent interruptions. May be asked to work beyond normal working hours, and in other positions temporarily, when necessary. Hourly Rate: $80/Hr.
