Laconia Nursing Home is seeking an experienced part-time RN MDS Coordinator. The primary purpose of this position is to coordinate the RAI process assuring the timeliness, and completeness of the MDS, CAA’s, and Interdisciplinary Care Plan. Responsibilities of MDS Coordinator: Monitors and guides the completion of assessments in a timely manner Involvement and supervision of inputting of MDS assessments, the transmission to the state, and obtains receipt of the validation Schedules and facilitates care plan conferences Facilitates monthly QM meetings Manages Care Planning Process Qualifications of MDS Coordinator: Must hold a Nursing Degree from an accredited college or university A current, unencumbered, active RN/LPN license and CPR At least 2 years’ experience in Long Term Care as an MDS Coordinator. Skills and Qualities of MDS Coordinator: Outstanding knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement. Excellent written and verbal communication skills and critical thinking abilities, Solid understanding of current federal, state, and local standards, regulations, and guidelines that govern our facility Ability to organize and prioritize. Laconia Nursing Home is looking for a Unit Manager to care for our Residents and facilitate their speedy recovery. The candidate will be part of the professional Nursing Administrative Team. The ideal candidate will be a responsible and well-trained professional able to give the best nursing care. The goal is to promote the patient’s well-being by providing high-quality nursing care. Responsibilities of Unit Manager: Monitor patient’s condition and assess their needs to provide the best possible care and advice Observe and interpret Residents symptoms and communicate them to physicians Collaborate with physicians and nurses to devise individualized care plans for patients Perform routine procedures (blood pressure measurements, administering injections, etc.) and fill in patients’ charts Adjust and administer patient’s medication and provide treatments according to physician’s orders Inspect the facilities and act to maintain excellent hygiene and safety (decontaminating equipment, sanitizing surfaces, preparing beds, etc.) Provide instant medical care in emergencies Supervise and train LPNs and nursing assistants Foster, a supportive and compassionate environment to care for patients and their families Expand knowledge and capabilities by attending educational workshops, conferences, etc. Requirements: Proven experience as a registered nurse In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination, etc.) and willingness to follow them at all times A team player with excellent communication and interpersonal skills Responsible and compassionate Valid nursing license (RN or LPN) -Must be Full Time
