Job Summary: The Registered Nurse (RN) provides holistic patient care service as a member of the primary care team. The RN performs routine clinical tasks necessary for efficient patient care and assists other patient care providers as needed. Essential Duties and Responsibilities: • Assess patient needs through problem-oriented history, including past medical history, current problems and vital signs. • Identify patients with significant health risks, including patients at risk due to hereditary factors, lifestyle, diet, substance abuse, social/cultural/environmental factors, and/or inadequate understanding of their disease process or treatment plan. • Perform patient education involving explanation to the patient of diagnosis, treatment plan (including use of medications and their effects and side effects), infection control (if indicated), and preventative techniques to lower patient risk of future disease or accident. • Assess patient needs through telephone interviews – advise patients over the phone and identify patients with urgent care needs. • Triage patients who arrive without appointments, including assessment and evaluation for need of medical care based on their medical history, present complaint, vital signs, and physical assessment (after physician consult if necessary). • Dispense medication under M.D. supervision and administration of oral, I.M. and I.V. medication as ordered by M.D., FNP, and CNW. • Case manage high-risk patients. • Facilitate patient/family group education sessions. • Coordinate patient flow and triage patients in the absence of the provider when provider is out or unavailable. • Update on-call providers daily on changing needs of complex, high maintenance or high-risk patients. • Maintain infection control within the clinic. • Perform laboratory procedures when necessary. • Perform other duties as assigned. • Handle protected health information (PHI) in a manner consistent with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) HIPAA Requirements: The RN will use PHI to answer patient questions about their medical problems or plan of care, to triage and schedule appointments for urgent medical problems, to relay information from the health care providers to the patient and to provider nursing visits at patients homes. Applying the minimum necessary rules of HIPAA, the designated record sets to which this employee will have access include: scheduling, demographics, and view only patient account information in the practice management system, and the complete medical record, including confidential sections. PURPOSE AND SCOPE: The registered professional nurse Home Therapies RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). Position Highlights: $41.66-$51.03/hour DOE with the ability to go higher for highly experienced candidates Additional pay for bilingual skills, paid on hours worked What You’ll Do: Provide comprehensive care during patient visits, discuss chief complaints, obtain vital signs, complete health history, and document health information in the EMR system Conduct phone triage to assess patient needs and coordinate follow-up care Assist with diagnostic procedures and treatments, including IV starts when ordered, while educating patients on procedures and medications Collaborate with the healthcare team to develop and implement patient care plans, coordinate referrals, and provide education to patients on medications and potential side effects Qualifications: Associate's Degree or higher in Nursing Oregon RN license Dual licensure in Washington and Oregon is required within 120 days of hire If an RN license is in a participating Nurse Licensure Compact (NLC) state, the following requirements apply: Must submit proof of license in state of residence. Must submit proof of completion for the required suicide prevention training during 1st week of onboarding. If relocating to Washington State, must obtain WA RN license within 6 months of hire. Must update the primary state of residence within 60 days (applies to anyone relocating to Washington State). Military Nurses are not required to obtain a Nurse Licensure Compact (NLC) BCLS certification within 90 days of hire; ACLS & PALS are preferred Skills: Bilingual (English/Spanish) preferred at level 10 Training in Early Periodic Screening & Development Testing and Women's Health Exams is preferred Proficient in professional nursing concepts and procedures, medication uses, and effects Strong verbal, written, and listening communication skills Ability to prioritize and multitask effectively, handling interruptions Skilled at interacting professionally with patients, families, and colleagues Able to assess and respond to diverse patient needs across all age groups Basic proficiency in Word, Excel, and EMR programs Our Mission Statement “Together we transform our communities through compassionate, individualized care, eliminating barriers to health and well-being.” Our mission celebrates inclusivity.
