RESPONSIBILITIES: As a Registered Nurse with Primary Care Solutions, you will enjoy the privilege of providing healthcare of the highest quality to America’s Veterans as part of a Patient Aligned Care Team (PACT) and will have an active role in developing and implementing a patient care plan. Office hours are Mon - Fri 8:00am to 4:30pm. This is a Full-Time position onsite. (no remote options) Assist practitioners with patient care and practice specialized nursing skills in patient assessment Triage urgent/emergent same-day patients for the team. Identifying acute patient needs and high acuity patients, identifying Veterans at risk for uncoordinated care, and mobilizing a care team for optimal patient outcomes Provide support by assisting our patients in navigating and coordinating their care Participate in a daily team huddle Answer and returns patient telephone calls, messages and alerts Provide patient and family education focused on self-management, prevention, and wellness Monitor discharge list and contact patients for follow up Establish and maintain a warm, welcoming, and professional atmosphere for our colleagues and veterans, ensuring needs and expectations are consistently met Promote achievement of clinical goals and metrics JOB REQUIREMENTS: 1 year of Primary Care and/or Chronic Disease Management experience Current, active, and unrestricted state Nursing License Current BLS for providers from the American Heart Association (to be renewed annually) Strong computer skills, including EMR experience. EDUCATION: Must be a graduate of an accredited program of nursing. Must have successfully completed additional theoretical and clinical education through an accredited graduate level family nurse practitioner program. REGULATORY REQUIREMENTS (IF APPLICABLE): Must maintain current Oklahoma licensure as a registered nurse (RN) and as an advanced registered nurse practitioner (ARNP). Must maintain current certification as a family nurse practitioner through American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP). PREFERRED QUALIFICATIONS: Experience as a family nurse practitioner is preferred but not required. PHYSICIAN ASSISTANT DEFINITION: The Physician Assistant is a health care professional, qualified by academic and clinical education, and licensed by the State Board of Medical Licensure and Supervision, who provides health care services in a variety of patient care settings under the direction and supervision of a physician. Patients cared for are infant, pediatric, adolescent, adult and geriatric. EDUCATION: Must be a graduate of a physician assistant program approved by the American Medical Association. REGULATORY REQUIREMENTS (IF APPLICABLE): Must be a licensed as a P.A. by the Oklahoma State Board of Medical Licensure and Supervision.
