The position will manage this patient population via telehealth in order to coordinate care with providers. The Triage Nurse position is based in Durham, NC at a non-clinical office site. This position interacts with patients via telephone and secure messaging vs direct patient contact. Job Duties and Responsibilities: Provide nursing triage, assessment and education to patients via telephone interactions and secure messaging. Manage patient needs including medication management, post procedure instructions and care coordination. Serve as a liaison to patients, family members and referring providers with guidance from pulmonary providers. Collect and analyze patient data, make recommendations for nursing care plans, evaluate nursing care provided, document and communicate appropriately nursing actions taken and patient responses.
