Education Associates in Nursing - Required Experience 2 Years of experience as a Licensed Practical Nurse in one of the following areas: Geriatric nursing; Community Health; Long Term Care; or Chronic Diseases of Adults. Completion of a course of study equivalent to a Bachelor’s degree in Human Services, Community Health or Gerontology may substitute for one year of experience in Human Services. License & Certification Active license to practice in the State of Georgia as Licensed Practical Nurse (LPN) without restriction – Required Certification provided by the Department of Community Health is required to be completed within 60 days from the hire date - Required Core Job Functions Complete in-home assessments on clients following appropriate guidelines and submits all assessments to a registered nurse for review and approval. Responsible for completing all required paperwork and data entry regarding the assessments timely and accurately. Understand the importance of patient satisfaction and excellence in customer service and how they affect the patient’s experience. Create and review comprehensive care plans based on the assessment findings initially and during annual reassessment to address patient risks/needs and establish goals for the patient. Demonstrate knowledge and skills necessary to identify patient needs. Assist with care coordination to achieving positive outcomes. Provide clinical guidance to the multidisciplinary care management team to achieve successful outcomes. Actively participate in care coordination planning and review with the multidisciplinary team while maintaining positive and productive working relationships. Provide care coordination to enrolled patients to include evaluation, care planning, monitoring of services, monthly contacts, and quarterly assessments.
