Lead quality improvement, practice change, and care redesign initiatives that enhance care coordination, improve access to specialty services, and promote safe, effective outpatient diabetes management. Mentor and consult with nurses and interdisciplinary teams on complex diabetes care, including medication management, diabetes technologies, insulin regimens, continuous glucose monitoring (CGM), insulin pumps, and patient self-management strategies. Support the integration of evidence-based practice, research, and new technologies while serving as a key resource for ambulatory nursing staff, diabetes educators, and organizational leaders across multiple practice settings. A minimum of two (2) years of recent adult diabetes and/or endocrinology nursing experience is highly preferred, with advanced knowledge of outpatient diabetes management, current diabetes medications, and diabetes technology strongly preferred.
