Charts nurse’s notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care. Completes the required forms upon admission, transfer, and/or discharge. Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted. Makes written and oral reports/recommendations concerning the activities of shift as required. Ensures that all aspects of resident care plans are implemented and maintained. Make recommendations for care plan changes to the IDT per professional standards. All other duties, as assigned. Licensed Practical Nurse (LPN) Skills and Abilities: Excellent written and verbal communication skills in English. Ability to multi-task and remain calm under stress. Analytical and problem-solving skills. Understands and follows infection control standards, policies and procedures. Must be able to adapt to a flexible work schedule and occasional overnight travel. Demonstrate basic computer knowledge and ability with an aptitude to learn company applications. Licensed Practical Nurse (LPN) Education and Experience: Current CPR Certification required. Certified Nursing Assistant (CNA) Duties and Responsibilities: Under the direction of the Director of Nursing or CNA Supervisor, the CNAs work closely with the nursing staff to ensure coordination of patient care. Provides physical support for patients/resident’s daily activities and personal hygiene, including bathing, dressing, grooming, transferring to/from bed, toileting, walking, standing, or exercising. Turns and repositions bedridden patients. Ensures proper resident/patient diet based on their dietary restrictions. Measures and records food and liquid intake. Examines patients for open wounds, bruises, or blood in urine. Notes and communicates patient/resident behavior and changes in mood. Gathers medical supplies. Cleans rooms and bed linens. Checks and records vital signs, including blood pressure, pulse, temperature, and respiration rate under the supervision of a licensed nurse. Answers to patient calls and needs. May aide patients being transported to and from appointments. All other duties, as assigned. Certified Nursing Assistant (CNA) Skills and Abilities: Good collaboration and teamwork skills with the nursing staff. Familiarity with symptoms and signs of improvement for a number of medical conditions. Ability to read, write, speak and understand English. Meets all health requirements, as required by law. Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers and other facility staff. Demonstrates basic computer knowledge and ability with an aptitude to learn company applications. Certified Nursing Assistant (CNA) Education and Experience: High School Diploma or GED. State Certified Nursing Assistant License/Certification. Current CPR certificate or become certified within 90 days of employment, preferred. Some facilities may require this. Physical Requirements: Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required. Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.
