Demonstrated ability to follow standard operating procedures and adapt in a fast-paced environment. Strong customer service and interpersonal skills, especially in high-volume or high-stress scenarios. Ability to work both independently and as part of a collaborative care team. We bridge the gap between chronic disease management and acute care needs, supporting members across various teams throughout the company. A bit about this role: This position represents an amazing opportunity for an Advanced Practice Provider ( NP or PA) with a big heart and entrepreneurial spirit who is passionate about chronic disease management. EDC, an integral part of Responsive Care, is the company's centralized hub for ongoing clinical management — a model built on a collaborative, shared patient management relationship with our members' primary care providers. In this role, you will work alongside a cross-functional team to deliver innovative care across Digital and traditional platforms. You will serve as a member of our ONE PCP team, as a prescriber of centralized prescription renewals and refills and also you will manage centralized results. You will utilize and help improve our home-grown technology and electronic health information platform to carry out virtual visits as well as managing asynchronous prescription refills for patients. Required skills and experience: APP with three or more years working in outpatient clinical practice ideally with experience in management of hypertension, hyperlipidemia and primary and secondary prevention of atherosclerotic cardiovascular disease. Minimum of 2 years of experience concentrated in primary care or a subspecialty with heavy focus on hypertension and lipid management required (e.g. cardiology, nephrology, neurology, endocrinology, primary care, urgent care). Licensure and certification For Nurse Practitioners: active and in-good-standing RN and APRN licenses. For Physician Assistants: an active and in-good-standing Physician Assistant license. Role licensure and certification in good standing is required and the ability to get licensed in requested states within 90 days of hire date, starting at offer letter signing. Your Responsibilities and Impact will include: Conduct focused and thorough assessments of patients with conditions that impact chronic health including hypertension, hyperlipidemia, coronary artery disease, and diabetes who are running out of medications. We expect that 40-60% of your clinical effort will be performing virtual visits. Formulate accurate diagnoses and develop individualized treatment plans for patients with chronic medical conditions, including medication management, lifestyle modifications, and monitoring recommendations. Manage a refill inbox for patients with chronic health conditions in accordance with established protocols and guidelines. We expect that 40-60% of your clinical effort will be spent managing a prescription refill queue. Participate in centralized results management workflows, ensuring timely review, triage, and follow-up on lab and diagnostic results across the patient panel in accordance with established risk-tiering protocols. Leverage Digital (live portal chat) and AI-assisted clinical tools — including agent-supported prescribing and inbox triage workflows — to improve efficiency, consistency, and safety in medication renewal, results management, and patient communication, while providing clinician feedback to help refine these tools as they evolve. Identify and evaluate risk factors, comorbidities and possible contraindications for treatment. Provide counseling on medication adherence, potential side effects. Collaborate closely with a cross-functional care team — including PCPs, specialists, and other the company team members such as pharmacy, clinical nursing, and social work — as well as interfacing with family members and caregivers as needed to coordinate holistic care for the member, to ensure continuity of care and deliver a collaborative care plan. Utilize our home grown electronic health information system for visits while also providing feedback on how to improve the interface. Participate in quality improvement initiatives and ongoing professional development to stay current on best practices and advancements in cardiometabolic care. Alamagordo Roswell Gallup Farmington Taos Las Cruces Grants Payrange:$50.00/hour Benefits Offered: Healthcare Dental Vision PTO 401K Your Job Summary The MDS Coordinator will be responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge and admission tracking, etc. With direction from the Director of Nursing and VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department. Principal Responsibilities • Works in collaboration with the Interdisciplinary Team to assess the needs of the resident; Provides interdisciplinary schedule for MDS assessments and care plan reviews as required by governing agencies. • Ensures that the Interdisciplinary team makes decisions for either completing or not completing additional MDS, assessments based on clinical criteria as identified in the most recent version of the RAI User’s Manual. • Assist with coordination and management of the daily stand up meeting, to include review of resident care and the setting of the assessment reference date(s). • Complies with federal and state regulations regarding completion and coordination of the RAI process. • Monitors MDS and care plan documentation for all residents; ensures documentation is present in the medical record to support MDS coding. • Maintains current MDS status of assigned residents according to state and federal guidelines. • Maintains the frequent and accurate data entry of resident information into appropriate computerized MDS programs. • Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident and family members. • Attends interdisciplinary team meeting, quality assurance and other meeting in order to gather information, communicate changes, and maintain and update records. • Assists DON or designee with identification of a significant change, physician orders and verbal reports to assure that the MDS and care plan are reflective of those changes. • Prepares scheduling, notice of resident care planning conferences, and assists DON in communication of outcomes/problems to the responsible staff, resident, and/or responsible party. • Continually updating knowledge base related to data entry and computer technology. • Completes electronic submission of required documentation to the state database and other entities per company policy. • Corrects and ensures completion of final MDS and submits resident assessment data to the appropriate State and Federal government agencies. • Assigns, assists, and instructs staff in the RAI Process, PPS Medicare, Medicaid (Case Mix as required) and clinical computer system in relation to these processes. • Maintains confidentiality of necessary information. • Other duties, responsibilities and activities may change or assigned at any time with or without notice. Qualifications • Graduate of an approved Registered Nurse program and licensed in the state of practice, required. • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. • Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required. • Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set. • Knowledge of the care planning process. • Experience with MDS 3.0, preferred.
