The nurse will work with a diverse patient population, including individuals living with HIV and members of the transgender community. The successful candidate must provide inclusive, compassionate care and be comfortable addressing the unique needs of this population. This position requires a highly autonomous and adaptable nurse who can work effectively with minimal supervision. Strong IV insertion skills are essential. Experience with PICC lines, midlines, line care and maintenance, wound care, and care management is strongly preferred. Community-health experience is helpful but not required. Participates and contributes to Performance Improvement, or Evidence Based Practice, or research at the unit level. Complies with standards and participates with unit level activities for regulatory compliance. Unit Operations Supports charge nurse decisions; performs charge role for assigned shifts as needed. Follows and supports the AACN guidelines for Healthy Work Environment (communication, collaboration, effective decision making, recognition, leadership, staffing); participates as team member focusing on elements within the Healthy Work Environment. Assists in the development of clinical practice protocols and standards for unit. Participates in unit based process governance activities: Performance Improvement, Professional Development, Clinical Practice, Teamwork, etc.; assists in development of action plans and supports implementation. Orients Clinical Nurse I to unit level governance structure and councils. Professional Development Serves as preceptor, coach, mentor to new nurses and students. Contributes to ancillary personnel skills and development, attends educational offerings and in-services independently. Contributes to development of unit based peer review processes; actively participates in individual peer review feedback Work Experience Education & Experience Licensure as a Registered Nurse in the state of Maryland, or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing, is required.; BSN preferred. Up-to-date status on all mandatory training and classes. One Year Nursing Experience. Solid Knowledge And Clinical Skills. Current CPR certification required. Maintenance of 10 contact hours of education every two years. Member in a professional organization is preferred. Recent applicable clinical experience in a comparable setting. About this Job: General Summary of Position Facilitates the inpatient admission of appropriate rehabilitation patients to MedStar National Rehabilitation Network (MNRN) by obtaining accurate clinical data through performing on-site and record/ telephonic patient evaluations. Individuals in this role are responsible for educating the referring staff of community hospitalspatients family members and external payers regarding the effective and efficient utilization of MNRN program services and resources. Primary Duties and Responsibilities Performs on-site record and telephonic pre-admission screening activities by gathering clinical information to support the evaluation of patients for admission to the inpatient rehabilitation program. Completes assessment/ evaluation within 24 hours or in one working day of the referral (documents exceptions within the electronic medical record). Reviews clinical information in patient's medical record and performs physical assessment of the patient within in the scope of LPN practice. Assists in compiling summaries and referral documentation for review by licensed provider responsible for determining medical stability and admission appropriateness. Reviews clinical data/assessment with MNRN manager and/ or physician for determination of medical stability and appropriateness for admission and notifies referral source of MNRN decision. Educates patient families or significant others referral sources hospital personnel external payers and case managers regarding inpatient acute rehabilitation program service and benefits admission criteria bed availability day treatment programs outpatient centers/programs and basic knowledge of third party reimbursement. Maintains marketing plans for each assigned hospital. Maintains a thorough knowledge of MNRN facilities products and services while developing and maintaining ongoing relationships with referral sources. Prepares/ submits marketing reports and all necessary data for accurate maintenance of statistics conducts tours for family members and referral sources and participates in formal marketing programs as directed. Participates in service area activities quality improvement projects and MNRN program steering committees. Provides feedback on admitted patients' progress at MNRN to referral sources as requested. Serves as a resource regarding rehabilitation services and admission processes within the scope of LPN practice and refers complex clinical questions or admission determinations to RN or Physician. Submits preliminary treatment plan estimated length of stay and rehabilitation goals required for precertification process. Communicates verbally with appropriate MedStar NRN staff regarding special needs e.g. equipment social issues etc. Keeps credentials needed for position current and active as required by governing practice body and MNRN policy. Travels via personal automobile with a 50-60 mile radius of MNRN to referring hospitals/healthcare facilities.
