Facilitate clear and effective communication between PACE patients, their families, primary care physicians, specialists, and health plans. Receive and process referrals from various sources, including agency staff, community organizations, and individuals seeking information about the PACE program. Assist with the enrollment process, providing comprehensive explanations of program details and ensuring potential patients fully understand their rights and the voluntary nature of enrollment.
