Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of 'direct' and 'indirect' patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. Promotes and maintain optimal client care under the supervision of an RN. Maintains a safe patient environment. Identifies and communicates significant abnormalities in patients data to case manager and other appropriate home health team members. Administers prescribed medications and treatments as permitted by law. Assesses all medicines a client may be taking to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, contraindicated medications and reports any problems to the physician. Re-evaluates client’s nursing needs and reports findings to Case Manager. Counsels and instructs the client and family in meeting nursing and related needs. Prepares clinical and progress notes. Submits all documentation in a timely manner in accordance with agency policy. Documentation on patients chart correlates with nursing care plan. Communicates frequently and works closely with other home care team members.
