About Legacy Hospice Services Legacy Hospice Services delivers high-quality, compassionate, home-based care to adults, children, and their families throughout the Portland metropolitan area and the Willamette Valley. We also provide hospice care in residential and assisted living communities, adult foster homes, and skilled nursing facilities - ensuring support and dignity for patients wherever they call home. Responsibilities Performs professional community health nursing visits and services for terminally ill patients and their families. Provides pain and symptom management tailored to patient needs. Serves as a member of the Hospice Interdisciplinary Team (IDT); coordinates and assists in implementing the plan of care for the patient and their family/caregiver. Completes nursing assessments and delivers palliative treatment and symptom management. May conduct initial visits to admit patients and initiate or revise the hospice plan of care. Prepares timely and accurate clinical documentation. Participates in IDT meetings and case conferences as appropriate. Collaborates with the care team to coordinate services and ensure continuity of care. Qualifications Education : As required by licensure. BSN or degree in progress strongly preferred. BSN may be required for specific assignments; experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO. Nurse Practitioner - Neonatal Specialist

Overview Position Overview Legacy Health offers an exciting growth opportunity to join a well-established team of collaborative and energetic Neonatal Nurse Practitioners (NNPs). This role provides the opportunity to practice across both Level III and Level IV NICU settings within the Legacy system. NNPs are integral to delivery attendance and the medical management of high-risk neonates and infants, including daily rounding on NNP service patients. In addition to clinical care, the role includes telemedicine resuscitation coverage for Legacy Level I nursery sites and opportunities to contribute to non-clinical initiatives that support the highest standards of care for NICU patients and their families. Level IV NICU – Randall Children’s Hospital The Level IV, 45-bed NICU is located within Randall Children’s Hospital, a freestanding, nine-story pediatric facility. This unit provides comprehensive care supported by a full range of pediatric subspecialties, including cardiac and neurosurgical services, genetics, and ECMO. Care is delivered within a family-integrated model, supported by a dedicated pediatric transport team. Notably, this role does not require transport responsibilities. Level III NICU – Legacy Salmon Creek Medical Center The Level III, 22-bed NICU at Legacy Salmon Creek Medical Center is a growing unit actively expanding its NNP team. Scheduling is flexible and evolving, with 12-, 16-, and 24-hour shift options available to support work-life balance and team coverage. Scheduling & Practice Model This position is based in the Portland/Vancouver area and requires dual licensure in Oregon and Washington. NNPs rotate between both NICU sites, with frequency tailored to individual preferences while ensuring 24/7 coverage, including nights, weekends, and holidays. A well-established self-scheduling model and structured holiday tracks contribute to provider satisfaction and balance. The NICU values input from all members of the team including dedicated neonatal RT’s, consultants from dietary, OT/PT, lactation, as well as the core nursing, medical and leadership staff. A variety of interventions including portable ventilators, multisystem trauma care, pain management and blood/blood products administration pre/post op care of adult neurosurgical patients including those requiring ventriculostomy management, may occur in this patient population. Responsibilities Responsible for patient care within an assigned department/unit and participates as a member of the health care team in cooperation with and under the direction of the clinical manager or designee as defined by the relevant Standards and Scope of Practice for the Registered Nurse. Responsibilities Provides patient care within an assigned department/unit Functions as a member of the health care team in collaboration with and under the direction of the clinical manager or designee Adheres to the relevant Standards and Scope of Practice for the Registered Nurse Performs all aspects of the nursing process: Assesses, analyzes, plans, implements, and evaluates patient care Follows established patient care standards set by Legacy Health System, including standards for patient assessment & patient transfer. Qualifications Education: As required by licensure. BSN or degree in progress strongly preferred. BSN may be required for specific assignments; experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO. Experience: One-year registered nursing experience or completion of either the Legacy Health RN Residency program or a nursing program that is equivalent in content, curriculum and duration. Additional clinical experience requirements may vary dependent upon the specific department in which the position is utilized. Skills: Demonstrated ability of positive interpersonal and communication skills with colleagues, patients, families, physicians and the community. Keyboard skills and ability to navigate electronic systems applicable to job functions. Licensure Current applicable state RN license required.
