About Legacy Hospice Services Legacy Hospice Services delivers high-quality, compassionate, home-based care to adults, children, and their families throughout the Portland metropolitan area and the Willamette Valley. We also provide hospice care in residential and assisted living communities, adult foster homes, and skilled nursing facilities - ensuring support and dignity for patients wherever they call home. Responsibilities Performs professional community health nursing visits and services for terminally ill patients and their families. Provides pain and symptom management tailored to patient needs. Serves as a member of the Hospice Interdisciplinary Team (IDT); coordinates and assists in implementing the plan of care for the patient and their family/caregiver. Completes nursing assessments and delivers palliative treatment and symptom management. May conduct initial visits to admit patients and initiate or revise the hospice plan of care. Prepares timely and accurate clinical documentation. Participates in IDT meetings and case conferences as appropriate. Collaborates with the care team to coordinate services and ensure continuity of care. Qualifications Education : As required by licensure. BSN or degree in progress strongly preferred. BSN may be required for specific assignments; experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO. Experience: One-year registered nursing experience or completion of either the Legacy Health RN Residency program or a nursing residency program that is equivalent in content, curriculum and duration. Hospice experience or one year medical/surgical acute care or long-term skilled nursing experience preferred. Skills: Working knowledge of and the ability to apply the basic principles and techniques of community hospice nursing in a variety of home settings. Must possess excellent clinical nursing skills and be able to utilize problem-solving techniques. Ability to establish and maintain effective working relationships with professional groups, volunteers and the general public. Must possess excellent communication skills both verbal and in writing. Overview Position Overview Legacy Health offers an exciting growth opportunity to join a well-established team of collaborative and energetic Neonatal Nurse Practitioners (NNPs). This role provides the opportunity to practice across both Level III and Level IV NICU settings within the Legacy system. NNPs are integral to delivery attendance and the medical management of high-risk neonates and infants, including daily rounding on NNP service patients. In addition to clinical care, the role includes telemedicine resuscitation coverage for Legacy Level I nursery sites and opportunities to contribute to non-clinical initiatives that support the highest standards of care for NICU patients and their families. Level IV NICU – Randall Children’s Hospital The Level IV, 45-bed NICU is located within Randall Children’s Hospital, a freestanding, nine-story pediatric facility. This unit provides comprehensive care supported by a full range of pediatric subspecialties, including cardiac and neurosurgical services, genetics, and ECMO. Care is delivered within a family-integrated model, supported by a dedicated pediatric transport team. Notably, this role does not require transport responsibilities. Level III NICU – Legacy Salmon Creek Medical Center The Level III, 22-bed NICU at Legacy Salmon Creek Medical Center is a growing unit actively expanding its NNP team. Scheduling is flexible and evolving, with 12-, 16-, and 24-hour shift options available to support work-life balance and team coverage. Scheduling & Practice Model This position is based in the Portland/Vancouver area and requires dual licensure in Oregon and Washington. NNPs rotate between both NICU sites, with frequency tailored to individual preferences while ensuring 24/7 coverage, including nights, weekends, and holidays. A well-established self-scheduling model and structured holiday tracks contribute to provider satisfaction and balance. Responsibilities Participates as part of the surgical team in assisting nursing staff in support services necessary for safe and efficient care of patients within the various areas of surgical services. Assist at the surgical field passing instruments and supplies to other members of the surgical team. Participates in acquiring, preparing and equipping the surgical suites. Assists in non-surgical patient intervention under the direct supervision of a physician or RN. May serve as a Core Technician, with responsibility for maintaining sterile technique and providing supplies and instruments as requested. Qualifications Experience: 1 year Surgical Technologist experience preferred. Education: Graduation from an accredited Surgical Technologist program or completion of a military training program for surgical technologists. OR High school diploma or equivalent, and As of January 1, 2017, in lieu of the formal Surg Tech training program requirement, may substitute professional experience if candidate has worked 2 out of last 3 years as Surgical Technologist. Skills: Basic level patient care skills per requirements of the position. Basic medical terminology. Keyboard skills and ability to navigate electronic systems applicable to job functions. Ability to understand and apply principles of sterile technique and surgical procedures within the scope of the position Able to work in a fast paced environment and prioritize multiple assignments. Flexibility and willingness to adapt to a changing environment and circumstances. Licensure All employees: American Heart Association approved BLS certification required at hire. Oregon Employees: Certified Surgical Technologist (CST) certification with the State of Oregon at hire, or within 12 months of graduation from an accredited Surgical Technologist program, required. The certification requirement will be waived for candidates who have completed an appropriate training program in a branch of the United States Armed Forces or the Commissioned Corps of the U.S. Public Health Service. The certification requirement will be waived for candidates who worked as a surgical technologist in an ambulatory surgery center (ASC), hospital or federal government agency between January 1, 2014 through January 1, 2017; provided that, the candidate has worked 2 out of the last 3 years as surgical technologist. Responsibilities Responsible for patient care within an assigned department/unit and participates as a member of the health care team in cooperation with and under the direction of the clinical manager or designee as defined by the relevant Standards and Scope of Practice for the Registered Nurse. The Vascular Access Nurse provides consultation for providers and nurses for the most appropriate vascular access device based on patient assessment and the treatment plan. Places vascular access devices such as: peripheral intravenous catheters (PIV), Peripheral Inserted Central Catheters (PICC), Midline catheters and Intra-osseous catheters. Accesses and de-accesses implanted ports. Assesses and manages central venous access devices including routine dressing changes, cap changes and intervenes to maintain patency of central venous access devices. Serves as a resource for the health care team in the intravenous access, placement, management, and patient/family education for care and maintenance. Service area is both inpatient and outpatient. Qualifications Education: As required by licensure. BSN or degree in progress strongly preferred. BSN may be required for specific assignments; experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO. Experience: One-year registered nursing experience or completion of either the Legacy Health RN Residency program or a nursing program that is equivalent in content, curriculum and duration. Thorough knowledge of care and skill in placement of peripheral IV catheters; thorough knowledge of care of central IV access devices and PICC competency as determined by Nurse Manager or designee.
