This role serves as the primary clinical resource, developing individualized care plans, managing symptoms, coordinating interdisciplinary services, and providing education and emotional support throughout the hospice journey. The RN Case Manager also provides functional supervision and direction to Hospice Aides and LPNs while ensuring quality patient outcomes and regulatory compliance. Key Responsibilities Conduct comprehensive patient assessments and develop individualized plans of care. Coordinate care with physicians, social workers, chaplains, aides, and other team members. Manage pain and symptoms using evidence-based hospice and palliative care practices. Qualifications Required Graduate of an accredited Nursing program. Current Ohio Registered Nurse (RN) license. Current CPR certification. Minimum one (1) year of acute care nursing experience.
