Description RN/Case Manager MSH Case Management PT Days The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of patients to determine the appropriateness of the Level of Care, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities 1. Principle Duties and responsibilities include, but are not limited to: a. Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. b. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. c. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. d. Documents all clinical reviews in Case Management Documentation system. e. Supports the mission, vision, philosophy and goals of the Medical Center. 2. Case Management: The CM process will include: a. Assessment of the patients clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. b. Identification and documentation of variances affecting the LOS and the discharge planning process. c. Conducts follow-up of any delays in treatment or reporting of results. d. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. e. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. f. Documentation of outcomes achieved and identified internal and external barriers. g. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. h. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. i. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. 3. Utilization Review: a. Maintains a working knowledge of the UR (Utilization Review) requirements of each payor within the patient population; b. Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. c. Provides clinical information requested by the managed care companies in accordance with contractual agreements. d. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Obtains authorizations from managed care companies for post-discharge services. c. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. d. Liaison with financial department for current insurance coverage. 5. Collaborates and participates in the appeals process with all members of the interdisciplinary team. 6. Collaborative Relationships: a. Develops and maintains effective working relationships with interdisciplinary team and Case Managers within the managed care organizations. 7. Participates in nursing orientation. 8. Interfaces with other departments in the Medical Center. Qualifications Licensing and Certification Requirements: Education Requirements Bachelors Degree in Nursing; Masters preferred. Experience Requirements Previous experience as in homecare, long term care or utilization review preferred. Discharge Planner or Case Manager preferred. Manager or a minimum of 3-5 years experience as a RN in an acute care setting Certification Requirements: Current NYS RN nursing license. CCM certification preferred. Under the direction of the RN, the Psychiatric Nursing Assistant (PNA) performs the activities related to the implementation of nursing care plans and the nursing standards, policies and procedures. The PNA is responsible for contributing to the development, evaluation and reassessment of care plans through observing and reporting changes in patient behavior and physical status. The PNA is also responsible for assisting in the maintenance of the safe and therapeutic environment of the unit. The individual is competent to care for patients in the following age groups: adult (ages 19 - 64) and geriatric (ages 65 +). Demonstrates an understanding of, and embraces, the mission statement of the Christian Health Care Center. Competencies: Supervises or assists in after meal care (removing trays, encouraging patients in brushing teeth, washing up, etc.). Serves food trays. Assists with feeding as indicated. Assists patients in preparing for activity and social programs, as per unit program schedules. Answers call bells promptly. Changes bed linens. Assists or supervises patients to walk with or without self-help devices as instructed. Reports all changes in the patient's condition to the Charge/Staff Nurse as soon as practical. Keeps floors dry. Reports spills immediately. Maintains intake and output records as instructed. Greets and escorts patient to his/her room. Informs the Charge/Staff Nurse of equipment and supply needs. Records all supplies and equipment utilized for appropriate procedures to Charge/Staff Nurse. Assists in transporting patients to/from appointments, activities and social programs, etc. as necessary/assigned. Maintains the confidentiality of all patient care information. Schedule: Part-time, 11pm - 7am, Working Every other weekend. Education: High School graduate or equivalent preferred. PATIENT THROUGHPUT COORDINATOR - RN

Responsibilities The Patient Throughput Coordinator will work with multiple entities to find and surmount the barriers to discharge of patient. This position acts as the designated liaison who facilitates and coordinates the transfer of patients into and within University Hospital. Works with referring agencies, hospitals, clinics, and the emergency department to ensure safe, timely transition of patients and clinically manages patient movement between units from the PACU, Emergency Department, outpatient clinics, procedural clinics and pre-admits for all service lines. Operating Room Technician I, (Certified Surgical Technologist) Perioperative Services

Responsibilities The primary purpose of the Operating Room Technician I position is to be under the direct supervision of the professional nurse. The OR Technician I performs various duties to assist with surgical procedures and related operating room activities. The OR Technician I delivers care as reflected in the Association of Surgical Technologists’ Recommended Standards of Practice in a manner that is cost effective without compromising quality of care. The OR Technician I possesses knowledge of the theory and application of sterile and aseptic technique and combines the knowledge of human anatomy, surgical procedures, and implementation tolls and technologist to facilitate a physician’s performance of invasive, therapeutic, and diagnostic procedures. The OR Technician I demonstrates competency to scrub for minor/major surgical and endoscopic surgical procedures. An Operating Room Technician I is a certified technician. Must be selfmotivated and work in an efficient and effective manner. Must work in air-conditioned environment with temperature of 65-73 degrees and humidity of 45-55%. When assigned to pediatric cases, temperature may be up to 85-90 degrees F. Must wear hospital provided scrub attire while on duty. Potential for cuts, bruises, muscle strains and exposure to contagious disease via and body fluids. Potential for exposure to anesthesia waste gases, ethylene oxide, harsh disinfectants, sterilizing agents, high steam pressure agents and radiation. Willingness to work with employees at various levels of skill, knowledge and abilities.
