This group consists of 11 attending physicians, 8 nurse practitioners, and a team of 14 clinical nurse coordinators. We serve patients at the following Siteman locations: Gary C Werth, North County in Florissant, South County at Hwy 55 and Butler Hill Rd, West County in Creve Coeur, and Shiloh, IL. This position will directly support providers in clinic during ambulatory clinic sessions and will perform administrative duties including patient care coordination, medication authorization, triage patient phone calls, ordering of laboratory and outpatient testing, patient education, and various Epic tasks related to patient care coordination. There is the potential for a hybrid work model upon completion of a satisfactory orientation period. As an integral member of the care team, they collaborate with other health care professionals to ensure continuity of care and coordination of a wide variety of patient care services.
