Lost Creek Healthcare and Rehabilitation Center is looking for an experienced Director of Nursing for our skilled Nursing Facility. A Director of Nursing is responsible to make a compelling difference in the lives of our residents through providing excellence and innovation in aging services. The Director of Nursing is responsible for strategic leadership, program development and implementation, and the overall administration of Nursing Services in accordance with community and resident needs, You must have exceptional knowledge of the state and federal regulations with a proven positive history with state surveys. Responsibilities: Oversight of the nursing floor, CNAs & LPN and Residents Oversees and coordinates the clinical operations of the facility implementing shared governance and relationship-based care in all departments as assigned by the Administrator. Oversees the safety and infection control programs for the nursing department. Provides guidance and leadership in meeting the CMS standards of quality. Ensures that the care plans, initial assessments, quarterly assessments, annual assessments, and documents of similar nature are filed in a thorough and timely manner. Maintain the highest possible quality measures of the facility to ensure that residents' are receiving the highest level of care. Provide supervision of CNA's, LPN's, and RN's in a supportive and clear manner. Qualifications: Graduate of an accredited school of Nursing. Valid State licensure as an RN Familiar with state & federal regulations. Must hold and maintain a current CPR certification Three (3) years of management or supervisory experience in long-term care Experience in supervising nursing staff and others for whom they are administratively or professionally responsible. Proficient in the use of personal computer including Microsoft Office Must be capable of maintaining regular attendance Must have the ability to complete multiple projects. Capable of working with minimal supervision. Must be able to maintain confidentiality regarding patient and company proprietary information. Must have the ability to relate professionally and positively and work cooperatively with residents, families, and other employees at all levels.

Scheduled Weekly Hours: 24 Work Shift: Nights (United States of America) Registered Nurse (RN) – Inpatient Rehabilitation – St. Rita's Medical Center Part-Time Nights 7pm-7am, every 3rd weekend Job Summary: The Inpatient Rehab Registered Nurse (RN) position is responsible for delivering exceptional nursing care to patients by: Utilizing strong organizational and leadership skills to assess patient status Implementing clinical nursing interventions Having a highly diverse medical skillset Developing and performing patient care plans to enhance and improve outcomes Ensuring a safe and therapeutic environment Essential Functions: Collaborate with the interdisciplinary care team Collect vital signs, administer IV medications, understand charting, and assess patient condition Assesses, stages, photographs, and measures wounds accurately and in accordance with policy Evaluates, updates and revises the plan of care to facilitate achievement of planned and expected outcomes Administer medications in a safe manner consistent with the State of Practice and the organization’s policies and procedures Other duties as assigned Education: Graduate of an accredited Associates degree or Bachelors degree in Nursing Certifications: Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) – American Heart Association Experience: 1 year of clinical patient care experience in an acute care setting, long-term care facility, or physician practice (preferred, not required) 1 year of experience in an acute rehabilitation facility (preferred, not required) Training: EPIC Electronic Health Record training (preferred, not required) Mercy Health is an equal opportunity employer. What You'll Do Administers medication (e.g., oral intramuscular and intravenous) and monitors patient medication compliance Performs treatments and performs or assists with nurse’s sick call Assists with doctor’s sick call Performs or assist with nursing management of chronic care clinics Responds to emergencies providing emergency care and cardiopulmonary resuscitation Performs subjective and objective assessment of patients Collects vital signs and timely documents all findings Collects urine and/or other bodily fluid specimens Prepares patients for release or transfer to other facilities (e.g., intrasystem or out to court) by reviewing patient care, transfer summaries or release summaries Ensures discharge medication orders are completed and timely forwarded to pharmacy services Participates in work-related training and institution training exercises.
