Coordinates the establishment of the initial plan of care with the attending physician, m edical d irector and the i nterdisciplinary t eam. Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of p hysician. Functions as a liaison between disciplines and facilities during the admission process. Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate. Communicates significant findings, problems, changes in condition or environment to the c linical m anager/ p hysician and other members of the hospice care team. Communicates with the p hysician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required. Performs on-call responsibilities and on-call services to patients/families as assigned. Performs other duties as assigned. Qualifications Required: Hospice or home health experience Current RN license in the state(s) where you will work Minimum 1 year of clinical RN experience in an acute healthcare setting Current CPR certification Preferred: Strong communication and interpersonal skills Ability to work independently and manage multiple priorities Comfortable with daily travel between facilities Proficient in electronic medical record (EMR) systems Ready to take the next step? Bay Area Direct Client Care has a contracted LPN position for 6 weeks dates Jan 7, 2025 -Feb 22, 2025. Days of the week are Sun, Mon, Tues 7am-7pm at $42/hr, weekly pay. This is at our medical group home in Ruskin. There are currently 4 consumers can admit 1 more with home health aide support. Clients are medically complex with tracheostomy, ventilator and feeding tubes. Will train (training rates apply at $25/hr).
