About this Job: Advanced Practice Provider (PA or Acute NP) Congestive Heart Failure (CHF) Clinic We are seeking an Advanced Practice Provider (PA or Acute NP) for our Congestive Heart Failure (CHF) Clinic at MedStar Good Samaritan Hospital in Baltimore, MD. Position Summary The Advanced Practice Provider (APP) in the CHF Clinic provides comprehensive, patient-centered care for individuals with congestive heart failure and related cardiovascular conditions. The APP works collaboratively with cardiologists, nurses, pharmacists, care coordinators, and other healthcare professionals to manage acute and chronic heart failure patients across outpatient settings. Responsibilities include patient assessment, diagnosis, treatment planning, medication management, patient education, and coordination of care to improve clinical outcomes and reduce hospital readmissions. Key Responsibilities Clinical Duties Conduct comprehensive patient evaluations, including medical history, physical examinations, and diagnostic interpretation. Diagnose and manage patients with heart failure and related cardiovascular diseases. Develop and implement individualized treatment plans in collaboration with supervising physicians and the multidisciplinary team. Prescribe and manage medications, including guideline-directed medical therapy (GDMT) for heart failure. Monitor patient progress and adjust treatment plans based on clinical response, physical exam, labs and evidence-based practice. Order, interpret, and review laboratory tests, EKG, imaging, and diagnostic studies. Manage urgent patient concerns and triage acute symptoms appropriately. Patient Education & Care Coordination Educate patients and families regarding heart failure management, medications, diet, exercise, and lifestyle modifications. Promote patient adherence to treatment plans and self-management strategies. Coordinate transitions of care following hospital discharge to reduce readmissions. Collaborate with community health advocate, pharmacists, and home health agencies to optimize patient outcomes. Participate in telehealth visits and remote patient monitoring programs as applicable. Quality & Compliance Maintain accurate and timely medical documentation within the electronic health record (EHR). Participate in quality improvement initiatives and heart failure performance metrics. Ensure compliance with clinic, hospital, state, and federal regulations. Support evidence-based clinical practice and participate in ongoing professional development. Qualifications Graduation from a Physician Assistant Program accredited by the Commission of Accreditation of Allied Health Education Program or Accredited Nurse Practitioner program Current state licensure as an NP or PA. Board certification as Nurse Practitioner or Physician Assistant. DEA registration and prescriptive authority as required by state law.
