Overview Hospice Clinical Liaison - RN Location: Atlanta, GA and some of Gwinnett County Find Your Passion and Purpose as a Full-Time Clinical Liaison RN Schedule: M-F / limited on-call What You Need to Know The Clinical Liaison, either a Registered Nurse (RN), or Social Worker, will act as liaison between the contracted hospitals and the hospice team, make inpatient visits to hospice patients in contracted facilities, act as consultant for pain and symptom management, provide in-service education to providers, perform pre-admission evaluations for appropriateness for hospice services, initiates the admission process by educating and informing patient/family of hospice and its services. Also obtain informed consents. Provide assessment, initial plan of care and intervention(s), which optimize patient and family directed comfort and quality of life and coordinates discharge planning. LPN or RN – In-Home Assessment Nurse (PRN/1099) – Metro Atlanta Gaddiel Home Care Services Inc. is seeking Licensed Practical Nurses (LPNs) or Registered Nurses (RNs) to join our team as in-home assessment specialists. As a 1099 contractor, your primary responsibility will be conducting 60–90 day client assessments in the comfort of their homes throughout the Metro Atlanta area.
