Makes note of any new medications, dosages, and discontinued medications. 6. Documents assessment entries into Harmony system within guidelines established by the agency and/or regulatory groups. Requests medical and hospital records as needed to assist with determining eligibility. 7. Participates in team meetings with Medical Director weekly to present all approved new admits, reassessments, hospitalizations, Home Health admits and discharges, and repeat variances. Also, members with potential for discharge, actual discharges, and situations threatening someone’s health or functional status. 8. Attends and participates in mandatory in-services. 9. Complies with corporate compliance program. 10. Reports job-related functions/tasks that involve occupational hazards including exposure to blood and body fluids and others as necessary. 11. Follows established safety regulations, to include fire protection & prevention, smoking regulations, and infection control and prevention. 12. Follow established safety procedures when performing tasks and/or working with equipment.
