Serves as liaison for all referral resources regarding appropriateness of referral, eligibility of services, community resources and home care issues and practices. Interprets Home Care policies to patients/families, hospital staff, and referral sources. Responds to telephone inquiries regarding information about home care services, makes appropriate calls to obtain referral and/or provides information for alternative community resources. Incorporates the clinical history, physiological and psychosocial of patients into the generation of a referral to insure appropriate discipline referral. Participates in in-services, continuing education, committees and interdepartmental meetings as assigned. Maintains and respects staff and patient confidentiality.
