About the Role: We are seeking Advanced Care Providers (Nurse Practitioners and Physician Assistants) to join our expanding Internal Medicine teams and collaborate closely with our Chief Medical Officers. Ideal candidates will demonstrate a commitment to providing high-quality patient care and bring an innovative, forward-thinking approach to their work. Candidates should possess strong internal medicine knowledge and have experience in urgent care, primary care, or family medicine, with a comfort level in treating the adult population. Essential Job Duties: Patient Assessment and Diagnosis: Nurse Practitioners and Physician Assistants (NPs/PAs) partner with CMOs to perform comprehensive health assessments, including medical history, physical examinations, and diagnostic tests, to identify patients' health status and needs. Diagnose acute and chronic health conditions, formulate differential diagnoses, and develop appropriate treatment plans. Treatment and Management: NPs/PAs collaborate with CMO to provide primary and specialty care services, including prescribing medications, ordering, and interpreting diagnostic tests, and implementing therapeutic interventions. Manage chronic conditions, monitor patients' progress, and adjust treatment plans as necessary to optimize health outcomes. Health Promotion and Disease Prevention: NPs/PAs educate patients and families about health promotion strategies, disease prevention measures, and lifestyle modifications to improve overall health and well-being. They emphasize preventive care, such as immunizations, screenings, and health maintenance activities. Collaborative Care: NPs/PAs collaborate with interdisciplinary healthcare teams, including physicians, nurses, and other healthcare professionals, to coordinate patient care effectively. They consult with specialists, refer patients to appropriate healthcare providers, and facilitate continuity of care across healthcare settings. Clinical Care: NPs/PAs will be required to perform clinical skills that include but are not limited to; phlebotomy, IV insertion, and EKG. May need to see patients independently for urgent visits on weekends. Documentation and Recordkeeping: NPs/PAs maintain accurate and up-to-date medical records, including patient histories, assessments, diagnoses, treatment plans, and progress notes. They will assist in completing any forms for patient care such as Radiology questionnaires. They ensure compliance with legal and regulatory requirements, as well as organizational policies and procedures. Ethical and Legal Compliance: NPs/PAs adhere to ethical principles and professional standards of practice, maintaining confidentiality, integrity, and respect for patients' rights and dignity. They comply with legal and regulatory requirements governing nursing practice, licensure, and scope of practice. Overview Under the supervision of the Nurse Manager, Critical care Nurses provide direct and individualized nursing care to patients based on the application of scientific nursing principles. Management patient post neuro procedure, or Neuro patient transferring from Surgical intensive care unit. Have knowledge of all vasoactive Drugs such as Vasopressors, Inotropes, Catecholamine, and Adrenergic. Calculate and administer according to the guideline. Monitors and adjusts specialized equipment used on patients, and interprets and records electronic displays, such as intracranial pressures, arterial line pressures, central venous pressures, pulmonary artery pressures, Balloon Pump, Hypo/Hyperthermia machine, CVVHD, Peritoneal Dialysis. Initiates patient education plan, as prescribed by physician. Teach patients and significant others how to manage their illness/injury, by explaining: post-treatment home care needs, diet/nutrition/exercise programs, self-administration of medication and rehabilitation. Demonstrates efforts in the prevention of hospital acquired conditions (HAC) CAUTI, CLABSI, Pressure injuries and Falls. Records all patients’ medical information concisely, accurately and completely, in a timely manner, in the appropriate format and on the appropriate forms. Prepares equipment and assists physician during examination and treatment/procedures of patient at the bedside. Participates in the formulation, review and or revision of departmental policies. Proactively participates in performance improvement initiatives to continuously improve the quality of patient care. Uses hospital resources effectively in providing patient care. Follows the process of reporting broken equipment. Recovering patient after sedation analgesia. Transferring patient to floor after verbal report/electronic report using Hand-off. Involves Patient and family in discharge teaching. Discharge patients safely if out patients. Performs other duties as assigned within the RN scope of practice. Qualifications Education: Internal & 1199 SEIU: Graduate of accredited nursing program(s). Bachelor's degree in Nursing preferred. Non 1199 SEIU Candidates: Bachelor’s degree in Nursing or equivalent to BSN program. Master's degree in Nursing, preferred. Experience: Minimum two (2) years' Telemetry experience required. POSITION OVERVIEW: The RN Case Manager is a direct link between medical services and the ICF/IRA residents and staff as well as with clinical professionals outside the program. The RN Case Manager is responsible for the complete nursing care and performance of all routine medical procedures within the scope of the nursing practice. The RN Case Manager is responsible for supervising LPN’s, AMAP and Direct Support Professionals (milieu counselors) in the performance of nursing tasks and activities.
