Experience caring for patients with chronic and complex medical conditions. Knowledge of inherited bleeding disorders, including hemophilia and von Willebrand disease preferred. Demonstrated experience with care coordination, case management, population health management, or transition of care activities. Experience providing patient and family education related to chronic disease management and specialty medications.
