ADMINISTRATIVE FUNCTIONS • Assist in planning, developing, organizing, implementing, and evaluating the recreation programs of this Facility. • Observe resident attendance, mood, behavior, and degree of involvement so that Facility activities and resident progress are evaluated, by noting, reporting, and charting resident behavior. • Participate in community planning related to the interests of the Facility and the services and needs of the resident and family. • Participate in discharge planning, development and implementation of recreation care plans and resident assessments. • Interview residents or family members to obtain recreation information. • Involve the resident and family in planning recreation programs, when possible. • Involve the resident and family in planning objectives and goals for the resident. • Refer resident and families to appropriate social service personnel when the Facility does not provide the services or needs of the resident. • Coordinate activities with other departments. • Keep abreast of economic conditions and situations and recommend to the Recreation Director adjustments in recreation programs that assure the continued ability to provide daily activities. • Assist in developing, implementing, and maintaining an ongoing quality assurance program for the recreation department. • Participate in Facility surveys (inspections) made by authorized government agencies. • Ensure that all charted recreation progress notes are informative and descriptive of the services provided and indicate the resident’s response to the service. • Arrange transportation for field trips. CARE PLAN & ASSESSMENT FUNCTIONS • Assist in developing a written plan of care (preliminary and comprehensive) for each resident’s recreation program that identifies the problems and needs of the resident and the goals to be accomplished for each problem and need identified. • Encourage the resident and family to participate in the development and review of his/her plan of care. • Follow the care plan in providing daily activities for the resident. BUDGET & PLANNING FUNCTIONS • Assist in the development of the department’s budget. • Purchase department supplies and clothing and personal items for the residents. RESIDENT RIGHTS • Review complaints and grievances made by the resident and make a written or oral report to the Recreation Director indicating what action(s) were taken to resolve the complaint or grievance. The Lakes Home Care is looking for a compassionate Home Health Aide to provide assistance in daily living and personal care services in patients homes in accordance with an established care plan. Responsibilities : Responsible for assisting patients with and per plan of care, the following: Baths, Oral hygiene, Dressing and undressing, Toileting activities, feeding, etc. Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency) Safe transfer and ambulation techniques Exercise according to the plan of care Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency) Patient’s laundry Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.) Observation and teaching of home safety practices Recognition of an emergency situation and knowledge of emergency management procedures Must follow patients’ plan of care and complete all tasks assigned Communicates in a constant, clear, and effective way with the client, family/caregiver and office Observes and documents patient condition and reports any changes, to the assigned nurse supervisor Requirements : Meets all training requirements in accordance with state and federal laws when applicable Ability to read, follow written instructions and document care given Possess and maintains current CPR certification in accordance with state regulation PPD or chest x-ray if applicable Successful completion of Maxim screening examinations Participation and completion of Maxim’s competency program Must meet all federal, state, and local requirements The Lakes benefits : part-time position.
