Coordinate and communicate resident care needs with physicians, families, pharmacies, and other care providers. Support health promotion, disease prevention, and wellness initiatives within the community. Ensure accurate, timely, and compliant clinical documentation. Assist with care planning, service plan updates, and resident evaluations as required. Collaborate with caregivers, wellness team members, and leadership to ensure consistent, high-quality care delivery. Respond to resident concerns, incidents, and emergencies using sound clinical judgment. Support training and guidance for care staff as appropriate. Qualifications: Current Licensed Practical Nurse (LPN) license, or a Professional Registered Nurse as per state regulations Minimum of two (2) years of Licensed Nurse experience required. Experience in assisted living, long-term care, or a skilled nursing environment preferred. Strong clinical judgment, problem-solving, and decision-making skills. Effective organizational, time management, and communication skills. Ability to manage multiple priorities and delegate appropriately. Demonstrated commitment to seniors and person-centered care.
