Obtains and maintains the integrity of specimens in accordance with policy and procedure, according to physician order. Provides for or assists patients with activities of daily living and fulfills designated and delegated responsibilities for ongoing continuum of care. Includes patient hygiene and changing linens. Provides one to one patient safety watch as assigned following established policies and procedures. Accurately processes specimens and provides timely transport of the specimen to the laboratory. Performs point of care patient testing and point of care instrument maintenance and quality control. Assists in problem solving, trouble shooting and taking corrective measures in care and maintenance of equipment and supplies. Serves as a preceptor and mentor for new staff and students in a positive manner. Performs clerical duties for admissions, transfers, discharges, and deaths according to standards and facilitates patient throughput activities. Transcribes and processes orders according to unit standard. Releases and confirms ordered tests to appropriate departments. Coordinate's transportation of patients to multiple tests. Participates in obtaining and communicating electronic information as pertinent to the nursing unit. Schedules tests, faxes consults, etc. Assists in ordering supplies. Stationery items as well as floor stock, patient needs, individual nursing supplies. Communicates with food services to ensure appropriate patient diets. Operates, problem solves, and takes corrective measures for clerical equipment. To include scheduling maintenance of computers, fax machines, phones, etc. Staff have access to non-controlled medications storage and supply areas and can transport non-controlled medications as necessary to meet the needs of the clinic or unit. Assists with transporting non-controlled medications to and from Pharmacy and within the unit as needed. Assists with transporting blood products to and from Blood Bank as needed. The RN CDI Specialist acts as a liaison between the clinical and coding functions. Provides education to the medical staff and other clinical professional on documentation relevant to the Revenue Management processes and Discharge Not Final Billed reduction. Provides daily interactions with physicians and clinical professionals regarding documentation clarification and optimization. It is expected that the CDIS have previous clinical skills, including an understanding of Anatomy and Physiology in order to appropriately discuss with the physician such issues as the underlying etiology, principal diagnosis, diagnostic studies, treatment modalities, to name a few. The essential focus of this position is to analyze the clinical information, using the documentation as the primary driver for overall System Case Mix Index. Position Summary: The purpose of the Licensed Practical Nurse (LPN) is to deliver care to residents utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation under the direction of the residents’ attending physician. Completes 24-hour report and gives thorough report to oncoming nurse. (including any calls that have been placed or need to be placed to physicians, RP’s, etc.) Observes and documents changes in residents’ condition and reports changes to RN supervisor; residents’ physician and resident’s RP if needed. Supervises and assists in bowel and bladder training program for residents, as needed. Able to organize work and prioritize. Receives medications and medical supplies daily and checks accuracy of orders received; logs narcotics appropriately; notifies RN supervisor of any discrepancies as soon as possible. Communicates with direct care staff with all resident care concerns at the beginning and throughout the shift. Answers families’ and visitor’s questions as necessary. Is responsible for emergency equipment and general nursing supplies; restocking medication/treatment carts and any other supplies used. Participates in care plan meetings when asked. Makes rounds with physicians. Reports all accidents and incidents to RN supervisor and documents on resident chart and on incident reports; informs doctor and family. Transcribes physician’s orders as per p/p; ensures all orders are written correctly and have appropriate dx. Monitors hydration of residents, including accurate I & O. Notifies dietician and MD if not meeting fluid needs or exceeding needs and documents same in residents’ chart. Makes rounds during mealtime to see that each resident has received help as needed, assists in hand feeding, assures that sufficient fluids, special feedings and nourishments are administered as needed. Completes assignments timely, i.e.: quarterly evaluations Coordinates activities with ancillary departments to ensure that treatments plans are followed and/or revised as needed. (i.e.: therapy) Utilizes equipment properly and ensures direct care staff does same; communicates to RN supervisor if education is needed. Monitors weight of all residents, utilizing protocol if there is a significant weight loss. Ensure plan of care is being followed. Cooperates with all staff; is respectful to coworkers. The RN Lactation Consultant is responsible for the planning, development, implementation and evaluation of an integrated lactation educational program. Coordinates and provides instruction for all medical and nursing staff. Works closely with team members such as, but not limited to, nurses, physicians, Social Services, feeding team, and other maternal infant clinicians to ensure patient educational needs are met. This union role is per diem; scheduled as needed. Hours typically fall between 7:00 a.m. and 7:00 p.m. Weekend coverage is also available. Certifications Required: Certified Breastfeeding Counselor / Certified Lactation Counselor or equivalent required; IBCLC International Board Certified Lactation Consultant preferred- must obtain within 2 years Job Duties: Develops protocols and programs supporting breastfeeding for all lactating mothers. Delivers prenatal education regarding benefits of breast-feeding to all clinic patients. Teaches breastfeeding classes and provides private consultations as requested by mother, nurse or physician. Maintains and organizes loaner pump program and resource center. Maintains affiliation with local support groups. Provides follow-up for all first-time breast-feeding mothers and infants or any couple needing special care. Continues with outreach programs in community and surrounding hospitals. Facilitates and promotes the integration of lactation philosophy amongst the staff including general philosophy of breastfeeding of the premature infant. Provides ongoing staff development related to lactation services. Attends patient care rounds. Participates in Performance Improvement in relation to lactation. Participates in research projects involving lactation. Education of staff on supplemental nursing techniques. Educates attending physician staff.
