We are seeking a highly skilled and compassionate Home Health Nurse (Registered Nurse or Licensed Practical Nurse ) to deliver exceptional in-home clinical care to patients in Gallup and Naschitti, New Mexico . This role involves providing individualized, physician-directed care in accordance with approved treatment plans while supporting patients in the comfort and safety of their homes. This is a 1099 independent contractor opportunity with an estimated 12 hours of weekly patient care. Current assignments include approximately 4 hours in Gallup and 8 hours in Naschitti. Once enrolled in the program, patients typically maintain long-term eligibility for services, allowing clinicians to establish meaningful, ongoing relationships and continuity of care. The ideal candidate will demonstrate strong clinical judgment, professionalism, and a patient-centered approach while traveling between service locations to provide high-quality nursing care. Responsibilities include patient advocacy, health monitoring, medication management, documentation, and collaboration with interdisciplinary care teams, including Certified Nursing Assistants (CNAs), Home Health Aides (HHAs), Case Managers, and physicians to ensure optimal patient outcomes. Complete compliant documentation of the care provided in the system of record. Manage and administer prescribed medication, treatment, and therapies. Conduct patient assessments, coordination of care. Perform various activities associated with daily living. Monitor vitals and GI intake and output. Assess skin integrity and administer wound care. Monitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progression. Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients. Provide education and training to the patient, their family, and/or the Home Health Aide. Work with patients to be done one-on-one in the patients home to provide for a comfortable environment. Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants. Perform other personal care services as necessary to meet the patient’s needs. Minimum Qualifications: Education, licensing, and certification: graduate of an accredited college or university, Associate or Baccalaureate School of Nursing Current State or Compact License as a Registered Nurse or Licensed Practical Nurse. Current CPR certification TB test and physical examination. Maintain required insurance. Good interpersonal skills. Hand-eye coordination and manual dexterity. Ability to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patients. Ability to operate under stressful conditions and make quick decisions. Must have dependable transportation and be willing to commute to the patient’s home. Ability to provide basic medical management up to medically fragile end of life care. Administers therapeutic measures as prescribed by the medical or surgical provider such as medication treatments, intravenous fluids, blood transfusion, oxygen therapy, etc. Records observations, nursing interventions, therapeutic measures, hemodynamic monitoring data, and other data relevant to patients in the unit. Responsible for independent management of patients undergoing local anesthesia and intravenous (IV) sedation. Provides specialized knowledge and skills to meet the physical, emotional, and spiritual needs of patients undergoing procedures. Operates and monitors specialized equipment pertinent to the unit.
