Efficiently prepare exam rooms by setting up proper equipment and supplies, following standard rooming workflows to prepare patients for provider exams, obtaining vital signs, health history verification, and ensuring accurate documentation in the EMR Clean exam rooms after each patient visit and maintain sufficient stock of supplies. Answer patient calls, obtain medical information, report test results, administer medications and vaccinations, prepare sterile fields, and assist with procedures as directed by providers. Perform therapeutic treatments, health screenings, specimen collection, Point of Care testing, and assist with splints or wound care under supervision. Respond to urgent situations, call for clinical assistance as needed, and assist in maintaining clinic medications, vaccinations, supplies, and equipment. Float to other departments or clinic locations as assigned by PHMG Leadership. Registered Nurse (RN) - Home Hospice and Palliative Case Manager -0.5 FTE- Days

What You Will Do: The RN Case Manager works collaboratively with patients, social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care/home hospice environment. Assesses, plans, facilitates and evaluates timely coordination of high quality, cost-effective care. Responsible for improving the health of patients dwelling in their own homes. This requires a nurse who is comfortable working independently, enjoys teaching, is a critical thinker and has strong communication skills. The RN Case Manager provides hands on care in the home environment and offers autonomy, flexibility and the luxury of caring for one patient at a time. Ideal candidates also possess negotiation skills along with strong analytical and computer skills. Experience with pre-acute and post-acute care as well as community resources is preferred. Case Managers also promote safety, cost efficiency and a commitment to quality improvement. What you will need: Associate’s Degree in Nursing required. Bachelor's Degree in Nursing preferred. Current State of WI or eNLC compact state RN license. Valid State of WI driver’s license with an acceptable driving record. Current BLS (CPR) certification or ability to obtain upon hire.
