Overview Part of CentraState Healthcare System - The Manor Health & Rehabilitation Center is a 123 bed sub-acute and long-term care skilled nursing facility. The Manor offers comprehensive short-term rehabilitation and long-term skilled nursing services for adults ages 18 and older. Affirms the Resident's right to make decisions regarding his/her medical care, including the decision to discontinue treatment, to the extent permitted by law. Demonstrates the ability to obtain information and interpret information in terms of the needs of the age of the Residents cared for. Demonstrates an understanding of the range of care needed based on the age of the Residents cared for. Demonstrates knowledge of growth, development and/or adult health throughout the life span as appropriate to the job performed. Successfully completes life-span education as appropriate. Accurately reports concerns or a change in Resident status to the RN/LPN. Provides partial and complete personal care to the residents. Assists in performing tasks to maintain resident mobility. Assists with Resident meal services and participates in the maintenance of the nutritional and hydration needs of the Resident. Verbalizes understanding that pain management is a priority to Resident and nurse. Notifies nurse immediately of Residents having pain/discomfort. Collects resident data utilizing learned technical skills. Assures Resident is safe prior to leaving them:- Has all items within reach- Call bell within reach- Alarms attached and working Restraints are applied according to care plan- Bed/ Chair proper level and location- Foot rests attached to wheelchair- Assures residents room is tidy prior to leaving. Informs resident and escorts to activity as necessary. Assists residents with toileting and has proper incontinent product as per care plan. Attends IDCP meetings regarding your resident (depending on shift). Assures C.N.A care plan is accurate and updates when indicated. Communicates ideas and observations. Promotes a positive attitude with residents and families. Continually improves quality of services provided. Qualifications High school graduate or equivalency preferred. Related experience preferred. Active NJ CNA (Certified Nurses Assistant) required. ED Care Manager (RN) - Days - Part Time - Benefit Eligible

Overview Emergency Department (ED) Nurse Care Manager is responsible for establishing, coordinating, and maintaining the process to optimize patient throughput to the most appropriate level of care while facilitating interdisciplinary care across the continuum. The Nurse Care Manager collaborates with the patient/family, multidisciplinary team, physicians, community resources and payers to ensure the patient’s progress and level of care is appropriately determined for the emergency episode of care and for the encounter to ensure that care is performed in the right level of care and facilitated in an efficient and cost-effective manner. Responsibilities Include, but are not limited to: Level of Care Determination: Completes initial medical necessity review on all ED patients with potential for admission, works with providers to identify appropriate level of care designation. Documents in accordance with established policy and procedure. Educates ED physicians and nurses about medical necessity/admission criteria and ensures documentation matches patient's clinical condition. Implements Actions to Prevent Readmission: Collaborates with medical staff to recommend referrals for outpatient care, home health or other community agencies as indicated. Requirements of the Certified Home Health Aide- MUST HAVE A VALID NEW JERSEY HOME HEALTH AIDE LICENSE! Certified Home Health Aides must be detail-oriented and be able to carefully follow protocols and rules regarding the patient’s care. Aides must adhere to instructions from doctors and nurses, such as knowing how to care for a minor medical issue or letting them know about changes in the patient’s health. They should be trustworthy and dependable so the patient and their family feel comfortable relying on them. Certified Home Health Aides should have the following skills and qualifications: High school graduate or G.E.D. certificate preferred. At least 1 year experience as a Caregiver/ Certified Home Health Aide in healthcare (in homes or facilities). NJ Licensed as Certified Home Health Aide a must Must be fully covid vaccinated & booster 2-step PPD Physical within the last 12 months Ability to read, write, speak and understand English as needed for the job. Possess a valid driver’s license and insured automobile Responsibilities of the Certified Home Health Aide- Certified Home Health Aides help patients in a variety of ways which may include taking their vital sign or giving them medication under a nurses direction. They must record and report this data as medically necessary. Certified Home Health Aides also can help patients with their daily functions, such as dressing or taking a bath. They can assist with planning for doctor's appointments, other meetings and may organize transportation for them. As a Certified Home Health Aide you will be able to do facility work, hourly as well as live in.
