Qualifications Education • Associate's Degree Graduation from an accredited school of nursing. Required • Bachelor's Degree Preferred Experience • Up to 1 year Preferred • Minimum of one (1) year acute care nursing experience. • One (1) year of experience should be relevant to specific department in which they will be working. • Refer to addendums for additional requirements. • Experience with Electronic Health Record Preferred. Overview The LPN is responsible for performing a wide variety of nursing care, patient activities, and accommodative services for the comfort and well-being of patients in accordance with physicians' orders and under the guidance and direction of the Registered Nurse. Responsibilities • Performs basic nursing care and patient procedures as required. • Participates in conferences and serves on committees as requested. • Observes, reports, and documents changes in patient condition, observations made, complaints voiced, and nursing care performed. • Participates in patient and family health education as necessary. • Provides a safe and secure environment for patients. • Prepares for and assists with diagnostic and therapeutic treatments. • May administer I.V.F. in accordance with the State Nurse Practice Act. • Checks, irrigates, and maintains drainage tubes as needed. • May insert N/G tubes after training (KY & VA). • May administer tube feedings. • Suctions patients according to hospital policy and procedure. • Functions in emergencies and on the code team as designated. • Performs catheterizations and provides catheter care. • Administers oral, intramuscular, intradermal, subcutaneous, I.V., P.B., and approved medications. • Administers I.V.P. medications as approved (Virginia only). • Cares for patients receiving oxygen therapy and analyzes oxygen concentration of O2 tents, isolettes, and croupettes. • Administers respiratory treatments (I.P.P.B.). • Applies sterile and unsterile compresses and bandages of all types and styles. • Answers call lights and assists patients with personal hygiene and activities of daily living. • Tests urine for sugar, acetone, and pH, weighs patients, and obtains vital signs. • Cares for patients in isolation. • Assists with physical and vaginal examinations and obtaining Pap smears. • Administers post-operative care, including coughing, deep breathing, turning patients, and obtaining vital signs. • Prepares patients for surgery, including completion of pre-operative checklists. • Applies ace bandages, all types of traction, hot water bottles, and ice caps. • Measures and records intake and output. • Collects specimens, including blood, urine, stool, and sputum. • Administers post-mortem care and transportation. • Administers enemas, including retention, cleansing, therapeutic, and colonic irrigation. • Maintains cleanliness, safety, and efficiency in nursing care. • Responds appropriately in emergency situations, providing patient care, initiating CPR, and performing duties designated by the RN or physician. • Assists with the care and security of patient valuables. • Checks narcotics and barbiturates, reports discrepancies, completes incident reports, and submits documentation to appropriate management. • Cares for patients with radium and assists physicians with removal of radium. • Prepares used equipment for re-sterilization. • Cleans and maintains storage of equipment and requests repairs as appropriate. • Follows up with physicians and staff regarding equipment performance. • Complies with hospital policies for obtaining, documenting, and following through on telephone and verbal orders. • Monitors glucose using a glucose monitoring device. • Collects Quality Improvement data as requested. • Uses universal precautions. • Performs other related duties as assigned. Qualifications Education • Graduate of an accredited school of practical nursing. Licenses & Certifications • Must maintain current licensure in the state of practice. • Must successfully complete the I.V. Therapy Course. Experience • Six (6) months to one (1) year of on-the-job training is required to attain proficiency in the performance of assigned duties. Training for all personnel assigned to the Emergency Room shall consist of the following: 1.State and federal regulations related to emergency services. 2.Nursing management of trauma. 3.Nursing management of cardiac related disorders. 4.Performance of EKG procedures, EKG interpretation, and appropriate nursing intervention. 5.Pharmacological intervention (IV therapy). 6.Patient assessment (triage). 7.Nursing management of pediatric emergencies. 8.Medicolegal aspects, particularly related to proper documentation. 9.Nursing management of obstetrical emergencies. 10.Patient/family emotional response. 11.Cardiopulmonary resuscitation and airway management. Qualifications Associate's Degree Graduation from an accredited school of nursing. Required Bachelor's Degree Preferred Up to 1 year Preferred Minimum of one (1) year acute care nursing experience (1) year of experience should be relevant to specific department in which they will be working. Refer to addendums for additional requirements. Experience with Electronic Health Record Preferred. Preferred Excellent interpersonal skills and ability to work with providers at all levels essential. Overview The Staff Nurse is accountable for interpreting the plan of Medical care, assessment of patients clinical decision-making regarding nursing care, assuring nursing care is provided in a safe and competent manner, providing individualized nursing care, and evaluating nursing care for groups of patients. Responsibilities Takes report from prior shift and gives report to oncoming shift which includes condition of patients, diagnosis, treatments, procedures given and required and other aspects of the plan of care. Performs assessments and reassessments of patients. Reviews ancillary service reports, communicates abnormal results to physicians, and notifies supervisor and physician when ancillary services are not completed. Interprets plan of medical care, evaluates patients, and devises patient plan of care. Maintains proper control of medicine, equipment and supplies specific to the assigned unit. Plans for discharge of patient considering all available resources and continuum of care needs. Refers patients to chaplain, social worker, and agencies as required. Provides a safe and secure environment for the patient. Implements the patient plan of care by assigning tasks to nursing and non-nursing personnel, and continuously assessing and evaluating patient care and patient's responses to nursing care. Performs nursing treatments and procedures and total patient care which may require expert skill and judgment in accordance with established nursing standards. May accompany physicians when possible on rounds, communicates pertinent information about patient to the physician, and assists the physician with treatments and procedures or delegate if not possible. Performs certain special procedures for which specialized training and supervised experience has been received. Plans for and provides effective patient and family education. Documents pertinent findings, plan of care and evaluation of care in the medical record. Assists patients in performing activities of daily living to meet the basic human needs. Prepares and maintains reports, charts, records, and forms as required by hospital, state, and federal regulations. May accompany patients who are transferred to another medical facility. Notifies supervisor, doctor, and/or family/guardian as appropriate when patient's condition changes. Observes and evaluates work performed by LPNs, Nursing Aides, ORT's and Clerks to validate that treatments and procedures are being performed in compliance with the plan of care and established hospital guidelines. Participates in teaching activities for other hospital personnel and nursing students. Participates in the orientation of new employees. Participates in committee meetings as requested. POSITION PHYSICAL REQUIREMENTS: Must possess physical abilities to endure prolonged standing and walking and ability to lift, carry, push and pull; exerting 50 to 100 pounds of force occasionally, and/or 25 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Heavy Work.
