We are seeking a dedicated Registered Nurse to join our Home Health team. to assume responsibility and accountability for the application of the nursing process and the delivery of patient care. The Registered Nurse (RN) demonstrates the ability to make clinical judgments in an effective and efficient manner under the direction of the Director of Clinical Services. Responsibilities Utilizes the nursing process to assess, plan, implement and evaluate patient care. Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition. The APRN is responsible for assessing physical and psychosocial status of patients in an outpatient clinic setting by means of interview, health history, physical examination, and diagnostic studies. Assist physicians in performing medical procedures, up to and including blood drawing, vital signs and nursing assessments, history, and physical. This position requires travel to multiple clinic locations. Collaborates with physicians regarding patients to assess, plan, implement and evaluate patient care. Provides direct care, counseling, and teaching to patients. Performs physical examinations and provides preventive health measures appropriate to patient needs. Develops, implements, and communicates plans of care in a timely manner to patient and family, as well as the appropriate team members, ensuring compliance with all regulatory guidelines. Facilitates staff, patient and family decision making by providing educational tools. Triages patient calls as needed to determine if patient needs to be seen or can provide guidance over the phone. Assess severity of patient's condition, determining and directing patients to appropriate medical providers. Provides telemedicine consultations or in person visits for established patients when provider is unavailable. Conducts pre-visit planning by reviewing schedule, charts, and outside medical records in preparation for next day visits. Actively participates in quality assurance programs and conducts peer-to-peer reviews with insurance carriers in support of patient’s medications. Maintains EMR inbox and closes encounters in a timely fashion according to department guidelines. Provides in basket message coverage for Providers as needed. Serves as a preceptor and assists new staff in the provision of care to help them acclimate to the healthcare environment, and a direct patient care role. Provides concise and constructive feedback when needed. Adapts to changing work demands and environment. Anticipates potential problems and assists nurse leader in the development of contingency plans. Interact with high profile patients in a professional demeanor.
