Position Purpose The Registered Nurse delivers high-quality, patient-centered care by applying clinical knowledge and expertise within the context of the nursing process. This includes individualized patient outcomes, coordination of care and educational support to patients. This role works in close collaboration with cardiology physicians and advanced practice providers (APPs) to enhance both patient care and the overall patient experience. Uses EPIC to document all chart and phone encounters, identifies potentially avoidable/non-reimbursed events, quality indicators and core measures. Maintains documentation, which is in compliance with regulatory guidelines and/or accrediting bodies. Maintains confidentiality of all medical and department records. Must be able to ensure all matters related to patient information are kept secured, meeting confidentiality compliance standards set by TJC and HIPAA. Creates a relationship-based patient experience for patients and clinical team. Improves the patient care experience. Coordinates patient communication and follows-up on a regular basis. Documents all patient contact in EPIC. Returns phone, email and MyChart inquiries within 24-48 hours or as specified by leadership team, keeping physician(s) and team informed about progress. The incumbent must be able to serve a variety of patients simultaneously, taking into account the unique needs of each.
