Job Description: Seeking a Clinic Care Tech, Medical Assistant, or a Licensed Practical Nurse to serve in Essentia Health's Hematology/Oncology clinic. This position will work Monday through Friday no holidays or weekends. Job Overview Fairview Range is hiring a Registered Nurse (RN) Case Manager to join our Home Care and Hospice Team in Hibbing, MN. The RN Case Manager in Home Care and Hospice plays a critical role in coordinating and delivering high-quality, patient-centered care to individuals receiving services in their homes or in hospice settings. This position is responsible for managing a caseload of patients, developing individualized care plans, overseeing implementation of clinical services, and ensuring that care is provided in accordance with medical standards, agency policies, and regulatory requirements. Serving as the primary point of contact for patients, families, and interdisciplinary care teams, the Case Manager facilitates communication, monitors patient progress, and adjusts care plans as needed to ensure comfort, dignity, and optimal outcomes. Compassion, clinical expertise, and strong organizational skills are essential in this role. The RN Case Manager coordinates the delivery of all the care/services appropriate to the needs of the patients of all ages and their families using an interdisciplinary team approach.
