Obtains and clarifies physician orders for plan of treatment revisions, informs physician promptly of significant changes in patient's condition, and provides written summary to physician within supplemental order. Provides effective and safe teaching using patient-centered care approach for patient and family to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals. Effectively and timely communicates with Naven Health’s Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines for billing optimization. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Naven Health’s policy. Under close supervision, coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Demonstrates compliance with agency operations, Naven Health’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule. The Day Center CNA ( Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care. Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting. Document care provided in accordance with established policies, guidelines, and applicable regulations. Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews. Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed. Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner. Collaborate with multidisciplinary teams in care planning for participants. Monitor appetite and look for changes in functional ability and skin condition. Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary. Report any variance, accident, or unusual occurrence to the appropriate personnel. Assist with loading and unloading of participants from vehicles. Assist the transportation department with participants’ take-home supplies. Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities. Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed. Attend in-service and continuing education programs as scheduled and necessary. The Certified Nursing Assistant (CNA) performs tasks essential to the direct care of the resident according to facility policy and procedures. What you will do Allows resident choices within limits of capability considering safety /dignity issues and follows policies and procedures of facility to include resident privacy and dignity. Assists with ensuring hygiene, appearance, safety and nutritional needs of residents are met. Provides resident care and services to maintain or improve resident functional status to highest practical ability. Documents accurately in EMR resident's ADL care, personal hygiene, bowel and bladder habits, and behaviors. Follows universal precautions, proper infection control, sanitation and safety standards of practice in all work and activities. Requirements: Diploma from an accredited Nursing Assistant/Aid required ONLY for new CNA grads within 120 days of program completion. Certification: CPR/BLS (within 30 days of hire) required. FA (within 30 days of hire) required.
