A caregiver who excels in this role will: Work collaboratively with a multidisciplinary care team across the continuum of care for high-risk patients to develop goals, plan interventions and maximize patient outcomes. Provide care and disease management coordination. Identify patients in the specialty care practice that have ongoing coordination needs and conduct targeted outreach. Outline the nature and duration of involvement needed by the specialty care team and specialty care coordinator and identify the primary care team involved. Utilize assessment skills and risk assessment tools to identify patients with actual or potential care needs that would require care coordination. Utilize technological tools (registries, patient lists, care team tab, etc.) to manage populations. Conduct comprehensive clinical assessments that include disease-specific, age-specific, medical, behavioral, pharmacy, social and end of life needs of each patient. Inform and work with patients and their families regarding coordination of their care, provide education and coaching, monitor patient compliance with their care plan, perform reassessments regarding patient progress toward goals and update plan of care. Serve as a liaison and advocate for patients and families. Assist in managing transitions of care across care settings, ensuring optimal communication and planning. Identify barriers, facilitate solutions and connect others to community resources. Partner with other care coordinator teams such as primary and transitional care social work, rehabilitation, pharmacy, palliative care and others. Define and ensure compliance with disease-specific care paths for specialty care or chronic disease. Coach patient and family on self-management support. Educate about managing a specialty or surgical condition (inclusive of preoperative, perioperative, postoperative and recovery) inclusive of prevention and health maintenance tasks. Educate and connect to other care providers and community resources to enhance care. Minimum qualifications for the ideal future caregiver include: Graduate from an accredited school of Professional Nursing Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross Three to five years of nursing experience Preferred qualifications for the ideal future caregiver include: Bachelor’s of Science in Nursing (BSN) Specialty certification Outpatient and/or Emergency Department experience Advanced Cardiac Life Support (ACLS) through American Heart Association (AHA) Certification Care coordination experience Physical Requirements: Requires full range of motion, manual and finger dexterity and eye-hand coordination. : Med-Tech, Full-Time Position Summary Medication Technicians are an essential part of our wellness team by administering medications to residents in a safe, accurate, and timely manner under the supervision of a licensed nurse and general direction of the Wellness Director. Shifts: 7am-3pm; 3pm-11pm and 11pm-7am. These shifts are availble full time and part time. Duties/Responsibilities: • Prepare and administer medications as allowed by state regulation to residents following established policies and procedures. • Accurately complete required documentation of medication administration at time-of-service delivery. Verifies identity of resident receiving medication and records name of drug, dosage, route, and time of administration on specified forms or records. • Review MARs immediately after administration of a medication and prior to end of shift to ensure documentation is complete and accurate. • Notify supervisor when making the decision not to administer a medication as ordered. • Communicate effectively at the end of shift relaying relevant and pertinent information regarding resident care, concerns, changes, and the like to oncoming/outgoing personnel. • Maintain security of medication cart and narcotic keys during entire shift. Conduct narcotic count at the beginning and end of every shift in accordance with company policy and procedure. • Observe residents and reports changes in resident’s psychosocial and physical condition as well as any concerns, e.g., skin breakdown, rash, behavioral changes, speech change, change in appearance, etc., to Wellness Director. • Maintain a clean, safe, and orderly environment for residents. Ensure resident rooms are clean and tidy throughout each day and follows scheduled housekeeping tasks such as laundry as assigned. • Assist residents with activities of daily living, to include ambulation, transferring, showering, personal hygiene, incontinence care, feeding, reminders, and supervision according to the individual plan of care. • Gathering, engaging, and assisting residents in activities, conducting activities in groups or one-on-one with residents as requested. Encouraging residents to socialize and participate in planned activities and develop friendships with other residents. • Communicates professionally with physicians and third-party specialists, such as homecare providers, lab providers and hospice professionals. • Maintain the resident record daily following company policy and procedure. • Follow established safety protocols, including fire protection, infection control, blood borne pathogens, hazardous waste, and quality assurance procedures. Intraoperatively, the RNFA functions interdependently with the surgeon and applies knowledge, skill, and judgment necessary to assist the surgeon in performing a safe operation that yields optimal results for the patient. The RNFA will provide patient care for all ages and/or provide leadership and direction to the perioperative team during the Perioperative phase as needed.
