Position Summary: We have an exciting opportunity to join our team as a Advanced Practice Provide Neonatal Nurse Practitioner (NICU) Level III Rotating Shift - Mineola. In this role, the successful Nurse Practitioner uses clinical expertise to plan the delivery of safe, therapeutic, population-specific patient care through assessment, planning, implementation and evaluation. With the patient and family central to the care, incorporates the unique characteristics and needs of each patient and family member into the plan of care. Works collaboratively within a multidisciplinary health team to promote wellness and optimize patient outcomes. Job Responsibilities: Performs other duties as assigned. Plans and organizes care to meet individual patient needs and to ensure appropriate clinical resource utilization per protocols, pathways, and other means. Works collaboratively with the interdisciplinary team including the attending physician to develop and implement the therapeutic plan of care for a group of patients and ensure communication with family members. Participates in the development and revision of the interdisciplinary and nursing standards, patient and family education materials as relevant to area of practice and expertise. Works collaboratively with the attending physician to maintain timely and appropriate communication with family members, house staff, other disciplines and ancillary services. Works collaboratively with nurse leadership to ensure timely communications with the nursing staff. Provides consultation about the therapeutic regimen to the nursing staff. Facilitates the continuity of care across the continuum of services, e.g. home care, other facilities, appointments with MD's etc. Demonstrates knowledge of the organization's Services Standards and incorporates them into the performance of responsibilities. Participates in designated activities related to professional nursing standards and regulations of relevant outside agencies. Functions as a respectful member of the healthcare team. Performs complete health history, physical examination and psychosocial assessment of patients within the designated practice or clinical unit. Orders customary laboratory, radiological and diagnostic studies per practice protocols, differentiates between normal/abnormal findings and follows protocols for abnormal findings, which may include referral to consulting physicians per customary pattern of the attending MD or with collaborating physician. Formulates the plan of care, along with the attending physician, patient and family, based on expected goals of care and length of stay. Works collaboratively with the attending physician, consulting physician(s), and other disciplines to identify, develop, and implement an appropriate plan of care that maximizes individual patient/ family preference and enhances quality, access, and cost-effective outcomes. Prescribes in-patient and discharge medications according to New York State law, including controlled substances with DEA authorization. Prescribes medical equipment, devices, physical and occupational therapy, and home health services per practice protocol. Practices as member of the allied health staff according to the rules and regulations of the medical staff and bylaws as outlined in the delineation of privileges. Evaluates treatment and health care plans for effectiveness and modifies per clinical standards and practice protocols. Considers needs and behaviors of specific patient age and cultural groups in all patient care. Promotes own professional growth and development in clinical and managerial role. Maintains current expertise in area of practice. Serves as a clinical expert and resource for the education of peers and other health professionals. Participates in the development and monitoring of patient outcomes per established practice protocols for purposes of quality and performance improvement. Uses evidence-based healthcare literature to advise and support appropriate practice changes within the designated service. Works collaboratively with nursing and other disciplines in the development and implementation of clinical studies within the area of expertise. Assists in the preparation, implementation, and evaluation of research protocols when applicable. Monitors and trends the cost effectiveness of the position, particularly as it relates to the efficiency and performance indicators established for service. For Ostomy, management of Ostomy patients in the outpatient setting. Minimum Qualifications: To qualify you must have a Master's Degree graduate of an accredited Nurse Practitioner program Required. Description A nurse practitioner is certified under section 6910 of the New York State Education Law and licensed as a registered nurse. A nurse practitioner provides acute/primary health care in accordance with written policies approved by nursing and medical administration. Responsibilities 1. Develops and implements a collaborative practice agreement in the specific clinical area and performs a periodic review of the collaborative agreement. 2. Description The (Per Diem) Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Assists in patient care and administrative duties necessary to deliver quality patient care. Takes and monitors vital signs, including fetal heart rate if necessary Performs phlebotomy, point of care testing, and other procedures as indicated by physician order, including but not limited to electrocardiogram and phototherapy Assists with procedures as necessary including but not limited to echocardiogram, sigmoidoscopy, proctoscopy, nasal scope, colposcopy, biopsy, LEEP, laser and cystoscopy Sets up oxygen and suction Performs vision screening Inventories, orders or re-stocks medical supplies; monitors expiration dates on all medications and medical supplies disposing appropriately as per institution policy Maintains logging system as required for refrigeration of medications, point of care testing, sterilization process, or others as needed Performs routine maintenance/cleaning of equipment and instruments; Notifies appropriate personnel regarding any malfunction in equipment Maintains a safe, secure, and healthy work environment by following standards and procedures; complies with legal regulations Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infections body fluids, chemical disinfectants, radiation and other hazardous substances Acts rapidly and calmly during emergencies, initiates CPR and provides the necessary items called for by the team Transports patients and equipment as needed Performs any and all duties involving direct patient care as assigned by the RN/LPN within the framework of the New York State Nurse Practice Act and MSHS Policy and Procedures Documents in medical record / electronic medical record as required by hospital policy Greets and registers patients, prepares and processes appropriate forms, directs patients to appropriate areas and arranges transportation needs Screens incoming calls, takes and responds to messages, as well as responds to inquiries regarding department procedures, services and programs. Schedules appointments and completes pre-registration via telephone including insurance verification and full patient demographics collection over the telephone or in person using computerized system(s) with accountability for the integrity of the data, including all patient demographics and insurance information Fulfills clerical responsibilities as assigned using the electronic medical record (EMR) system or manually as necessary, to include sending and receiving patient medical records; obtaining lab/x-ray reports, discharge summaries, referral information, etc. Performs routine cash collections at time of service. Completes forms and requisitions as needed, managing charts to ensure information is completed and filed appropriately. Assists in the creation and maintenance of medical charts. Retrieves lab, pathology and diagnostic testing, and operative reports and data from computer systems Checks, verifies and updates insurance information utilizing internal or electronic sources. Obtains and documents managed care referrals, required pre-certifications and authorizations as necessary Supports surgical or procedure scheduling in conjunction with Surgical Coordinator or other similar role(s) verifying times with patients, preparing charts, pre-admissions and consent forms Maintains a working knowledge of New York State third party payer eligibility requirements for private and hospital billing Utilizes and understands basic medical terminology; able to complete visit encounter forms and enters ICD10-CM and CPT information into computerized systems as appropriate Assists in training, orientation and education of entry level and or/float staff Qualifications High School Diploma/GED. Description Position Title: Licensed Practical Nurse-Institute for Advanced Medicine-Mount Sinai West-Full Time-Days Position Summary: Participates in the application of the nursing process to an assigned group of patients. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse. Responsibilities A. Clinical/Technical/Service: Participates in the application of the nursing process to an assigned group of patients. Demonstrates those behaviors which reflect the scientific knowledge, judgment, and technical skills of a Licensed Practical Nurse Performs job duties under supervision of a Registered Professional Nurse. Demonstrates a competent level of approved skilled nursing care tasks and procedures using approved technical skills related to vital signs, pulse oximetry, venipuncture, starting standard peripheral IVs only (not midlines or long peripheral lines), urinary catheter insertion/catheter care; and simple dressings, remove/discontinue a peripheral line. Administer IV fluids, provided completion of annual competency IV therapy training. Monitors patients receiving blood and blood products and TPN under the direction of the RN, who administers the transfusion. Makes recommendations in the application of the nursing process. Participates in planning, and administers direct patient care, including activities of daily living (ADL). Performs point of care testing including blood glucose. Performs nursing procedures as defined in the nursing procedure manual. Administers medication as defined in nursing policy. The LPN will participate in educational programs on a regular basis, including initial orientation and continuing in service training to care for patients with central venous access devices (CVAD). The training should include a written examination and competency demonstration. A Hemodialysis LPN may: • Administer bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first dose administration which is administered by an RN. The RN will access and care for patient’s initial catheter use. Subsequently, and following the MSHS CVAD care policy, the LPN can: • Identify and set up supplies necessary for accessing a CVAD. • Initiate treatment with a CVAD. • Draw blood specimens from a CVAD. • Perform CVAD dressing change and exit site care. • Monitor and adjust flow rates of central venous catheter access device/line. • Intervene if there is a catheter/blood line disconnection or other complications associated with a CVAD. • Provide CVAD care at the end of dialysis treatment. Qualifications Provides documentation of completion of an accredited LPN training program Documented previous work experience or educational preparation to support position assignment.
