The Division of Family and Child Health (DFCH) of the New York City Department of Health and Mental Hygiene is charged with the creation and oversight of programs, policies, services, and environments that support physical and socio-emotional health, and promote primary and reproductive health services, health equity, social justice, safety and well-being for New York City families and children. The Division is comprised of the Bureau of Maternal Infant and Reproductive Health, the Bureau of Early Intervention, the Office of School Health, and the Bureau of Administration. DUTIES WILL INCLUDE BUT NOT BE LIMITED TO: Work within the mandates, policies and protocols of the Office of School Health. Support all clinical initiatives put forth by the OSH, with a particular focus on the Connecting Adolescents to Comprehensive Health Care (CATCH) Program. Provide a full range of sexual and reproductive health services including, but not limited to, pregnancy tests, contraception, counseling and education. Provide physical examinations for sports, working papers and new admission requirements. Independently examine patients; formulates diagnoses; directs or performs appropriate therapeutic treatment and counsels patients; offers recommendations on care and treatment. Improve the health of school children through case management of chronic disease, preventive health screening and counseling, health education, and referrals. Utilize the Automated Student Health Record (ASHR) to maintain adequate student health records. Support the development of school educational and prevention programs promoting the health and wellness of all students. Provide trainings/presentations to school staff, community organizations, or parents. Assist in administration of staff training and orientation for new Connecting Adolescents to Comprehensive Healthcare (CATCH) nurses. Assist with clinical nursing services for students as indicated. Stay abreast with policy statements, research and medical advances developed within the health community. Assist with developing nursing clinical policies and practices around reproductive health services. Serve in a leadership role with OSH staff in medical room. Participate in OSH research and quality improvement activities. Participate in all mandated DOHMH and OSH trainings and professional development sessions. Provide clinical assistance in the event of an environmental or communicable disease occurrence. Preferred Skills: -A valid NYS license as a Pediatric or Family Nurse Practitioner with a minimum of 3600 hours of licensed NP experience. -Clinical experience in family planning and adolescent health. -Knowledge of current family planning standards of practice. -Familiarity with best practices in working with adolescents. -Effective communication and interpersonal skills. -Computer skills in Microsoft Office. -Experience with Electronic Medical Records. -Ability to maintain good public relations and to communicate effectively with students, other clinical providers, school staff and parents. -Ability to have sensitivity and flexibility when working with groups of various backgrounds. -Ability to travel to various school locations across NYC. Clinical Care * The Physician Assistant (PA) or the Nurse Practitioner (NP) provides assessment, triage, and therapeutic management of patient/family needs throughout the disease process as an integral part of an interdisciplinary team. Information Management * The NP/PA ensures appropriate documentation within the accordance of hospital and regulatory requirements and protects information of all forms, whether computer-based, paper, film, voice, or other media from unauthorized access modification, destruction, or intentional or accidental disclosure. 4. Quality Management * The NP/PA is expected to demonstrate a high standard of moral and ethical behavior, professionalism, compassion, and commitment to patient care. * Ensures compliance with patient related healthcare regulatory requirements, standards and recommendations. 5. Qualifications EDUCATION Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program. EXPERIENCE Prefer clinical working experience as a NP/PA within the given specialty/service line recommended, but none required. APRNs, PAs, CRNAs and CNMs are required to be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process. Evidence of current competence to practice as {an APRN, PA, CRNA, CNM} via appropriate reference letters from physicians and other practitioners must be able to be obtained during the Medical Staff appointment process in order to be eligible. Please note this is a RN phone triage position located in our administrative office. Contact with patients will be via phone, not face to face. Job Duties & Essential Functions: Accurately collects clinical data on patients and demonstrates competency in performing clinical interventions and reports abnormalities to Medical Provider. Triage patient phone calls and troubleshoot medication management issues. Demonstrates competency in answering phones, assembling charts, patient registration, making appointments, computer data entry and retrieval of reports. Must safely perform all patient care responsibilities in accordance with professional standards and the organization’s policies and procedures. Develop and organize a plan of care in consultation with treating physician, as indicated within the various subspecialties of the Department. Must effectively determine and set priorities to meet patient needs. Provide proper and accurate documentation of patient care in the patient medical record. Coordinate any services or accommodations necessary (e.g. sign language interpreter) to ensure effective communications between treating providers and patients who are hearing and/or visually impaired, and patients with language barriers. Ensures that patients receive timely and appropriate care within the delivery system and facilitates internal and external referrals. Administer medications, treatments, and contrast agents according to the specific orders of the ordering physician, and document in the patient record. Maintains infection control precautions and identifies potential infection control problems. Provides in-service education to members of the interdisciplinary team within the Department. Participate in Quality Assurance activities within the Department. Participates in annual recertification training, compliance training, and HIPAA training, as well as all other mandatory in-service programs. Conforms to policies and regulations governing patient rights and confidentiality of information. Perform all other related duties as assigned by management. Qualifications Required Education & Qualifications: The candidate must have an Associate’s degree in Nursing. (New York State BSN in 10 Law - Must attain Bachelor’s in Nursing or higher within 10 years of initial licensure. Current RNs, as well as those currently enrolled or pending acceptance into a program preparing registered nurses effective date of this act (which is Dec. 19, 2017) are grandfathered in.) Basic Cardiac Life Support (BCLS) required. Current registration to practice as a Licensed Registered Nurse in New York State. Must have excellent communication skills. Preferred Qualifications: Bachelor’s degree in Nursing. Advanced Cardiovascular Life Support (ACLS) preferred. Triage patient phone calls and troubleshoot medication management issues Documents nursing observations, interventions, and patient’s response to treatment in patient records. Demonstrated competency in performing venipuncture/administering shots for collection of blood specimens and facilitates the prompt and accurate processing and distribution of specimens collected. Ensures that patients receive timely and appropriate care within the delivery system and facilitates internal and external referrals in collaboration with the Registered Nurse (RN). Accurately performs and records vital signs, weight and height, point of service blood glucose testing, and promptly reports abnormalities to the medical provider or Registered Nurse (RN) on duty. Utilizes appropriate techniques when collecting routine urine specimens and specimens for C/S. Appropriately instructs patients on clean catch specimens. Utilizes appropriate technique in performing venipuncture to obtain blood specimens in accordance with agency procedure. Verbalizes knowledge of and adherence to OSHA standards for blood borne pathogens when handling blood and body fluids. Maintains standard precautions in the provision of patient care. Maintains and monitors stock inventory in the treatment center and replenishes supplies in exam rooms on a timely basis. Prepares and processes requisitions for routine supplies in accordance with agency procedures. Monitors the integrity of equipment and promptly reports malfunction, missing or damaged equipment to the Registered Nurse (RN) or Office Manager. Demonstrates competency in answering phones, patient registration, making appointments, computer data entry and retrieval of reports. Copies, prepares records, and stores and maintains appropriate records. Keeps records of all laboratory work sent, files laboratory reports on client record, and prepares and processes Lab/Diagnostic requisitions.
