As a HouseCalls Nurse Practitioner you will conduct annual in-home health assessments for Medicare Advantage and other plan members. This is a non-prescribing, field-based role focused on improving health outcomes through education, gap closure, and collaboration with primary care providers (PCP). Primary Responsibilities: Conduct comprehensive in-home assessments, including: Past medical history review, medication reconciliation, vital signs, and physical exam Evidence-based screenings and point-of-care testing (as appropriate) Identify and document diagnoses for care management and treatment planning Communicate findings to members' PCPs to address gaps in care Recognize urgent/emergent situations and intervene appropriately Educate members on disease processes, medications, and compliance Address social determinants of health and provide referrals as needed You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Responsibilities We invite you to join our highly engaged team of Specialty Nurse Navigators. As a Specialty Nurse Navigator, you will be responsible for: Navigating patients through the continuum of care acting as a liaison and resource between the physician, other departments, and the patient. Establish specific, measurable, attainable, relevant and time specific nursing goals to guide the care planning process and promote self-reliance. Provide in person, clinic visits as needed at the initiation of care and throughout transitions of care throughout all of our hematology/oncology clinic sites, as needed. Formulating custom care plans from assessment and clinical information demonstrating the application of clinical guidelines including standards of care. Providing disease and treatment specific education tailored to meet patient’s individual goals of care. Onsite rounding throughout treatment plan implementation as needed to provide needed in person patient evaluation. Provide direct, hands-on nursing care, as needed. Assist patients in establishing, modifying, or reinforcing a social support network, thereby promoting self-reliance. Assisting patients with social services needs by identifying resources in the community and referring to these programs. Collaboration with system wide authorizations team, scheduling in other departments, as needed to ensure patient receives timely and comprehensive care. Required Education/Experience/Specialized Skills BSN plus 3 years of recent clinical nursing experience in a variety of clinical settings. Required Certification/Registration CA RN License BLS if working in a clinical setting. ONS Chemotherapy and Biotherapy. Preferred Education/Experience/Specialized Skills/Certification MSN. OCN. Ambulatory experience preferred. Minimum of two years of outpatient experience managing the care of patients in oncology or a related specialty. Hematology experience.
