Communicates plan of care in a timely manner to patient and family, as well as the appropriate team members, ensuring compliance with all regulatory guidelines (i.e. HIPAA). Uses best practices for transition of patient care. Uses available resources to assist in discharge planning. Safely operates medical equipment. Serves as a preceptor and assists new staff in the provision of care in order to help them acclimate to the healthcare environment, and a direct patient care role. Department Specific Functions Triage and Prioritization- Responsible for triaging and prioritizing all patient appointments and messages for clinicians and nurse practitioners. Expedited Appointments- Facilitate expedited appointments for patients requiring urgent care, considering the long wait times for the next available slots. Symptom Assessment- Perform assessments of incoming patients to evaluate symptoms and ensure appropriate triage. Family Communication- Communicate with patient families to provide guidance and establish an appropriate course of action. Call Triage and Symptom Management: Assess and prioritize patient needs over the phone. Determine the urgency of care based on a brief evaluation.
