CORE JOB SUMMARY The Nurse Navigator 2 Educates patients and their families about their diagnosis and assures timely scheduling of the first appointment, and coordination of care after, and supports the patient throughout the care continuum. DEPARTMENT SPECIFIC JOB FUNCTIONS Manage and respond to Interventional Radiology Advanced Practice Provider (APP) communications including emails, phone calls, and secure chats, ensuring timely follow-up and coordination of patient care activities. Facilitate peer-to-peer reviews and prepare comprehensive appeal letters to support authorization and coverage for medically necessary procedures. Conduct outreach to new patient referrals via phone and email to coordinate and schedule visits, ensuring a seamless onboarding and care navigation experience. Request and review external medical records and imaging for appropriate scheduling. Participates in coordination of the plan of care with the multidisciplinary team, promoting timely follow-up on treatment and supportive care recommendations Communicates with APPs and physicians as needed by phone, emails or in person for new patient referrals and scheduling priorities Must be able to perform job duties as an independent professional and as a team player; organizational skills sufficient to set own priorities and facilitate work team progress.
