Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. DSD Services teammates will have access to all the capabilities and resources DaVita has to offer, and will work to quickly leverage these to develop the infrastructure and rapidly grow the new service line. Essential Duties and Responsibilities: Deliver nursing assistance to patient utilizing each patient’s physical, psychological, cultural, nutritional, and functional needs under the supervision of the registered nurse Completes patient care timely and accurately based on the prescribed plan of care in conjunction with the Registered Nurse Completes documentation according to specific hospital policy Performs clerical / receptionist functions on the nursing units Promotes a clean and safe environment of care, utilizing the SAFE error prevention habits Provides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct Required Qualifications : Education and Training : Currently certified by the state of Illinois as a CNA. High school diploma or equivalent preferred . Must be 18 years of age or older . CPR certified. Excellent customer service skills. Ability to care for patients with compassion and maintain quality in a fast-paced environment . Previous hospital experience strongly preferred.
