More about our team We are a 12-bed inpatient rehabilitation unit and the only acute inpatient rehab option in the area, serving a diverse population with a wide range of diagnoses. Our team practices a strong interdisciplinary approach, allowing nurses to work closely with therapy, physicians, and case management to deliver comprehensive, patient-centered care. We maintain a supportive work environment with an average nurse-to-patient ratio of 4-5:1, promoting quality care and professional growth. How you'll contribute A Rehabilitation RN who excels in this role: Provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice. Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment. Documents patient care given. Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family. Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation. Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs. Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances. What we're looking for Applicants should have an Active Tennessee license (single-state or compact) and possess an associate's degree from an accredited nursing school. If relocating, licensure must be updated to the new Primary State of Residence (PSOR) before employment begins. Additional requirements include: Certifications: AHA Basic Life Support (BLS) within 30 days of hire. Required Skills: Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Reporting directly to the Administrator or Executive Director, you will play a vital role in delivering hands-on hospice care to patients and families in their homes, assisted living, or facility settings. As a Hospice LPN, You Will: Provide direct nursing care services to hospice patients, supporting both patients and their families. Assist the interdisciplinary team in assessing patient/family needs and contribute to the execution of individualized care plans. Report any changes in patient conditions promptly to the Registered Nurse and care team. Deliver empathetic care aligned with each patient’s unique plan of care.
