We are seeking a Nurse (RN) Case Manager : Bring a strong clinical background, case management experience and well-developed assessment and critical thinking skills Assist with anticipating the discharge needs of post-surgical patients to ensure timely discharge and access to the best possible resources Have an understanding of medical necessity, level of care and patient status (inpatient vs outpatient). Knowledge of community resources such as transportation, skilled nursing facilities or home health agencies. Enjoy learning, asking questions, and working within a collaborative team environment. Equivalent combination of education and experience will be considered. Required Master's Degree Master’s degree in Nursing or healthcare related field Preferred Work Experience 3 years Three (3) years of relevant clinical nursing experience.
